正骨夹板外固定和手术内固定治疗高龄骨质疏松性桡骨远端骨折的效果比较

Comparison of the Effects of Bone Splint External Fixation and Surgical Internal Fixation in the Treatment of Senile Osteoporotic Distal Radius Fractures

目的比较正骨夹板外固定和手术内固定治疗高龄骨质疏松性桡骨远端骨折的临床疗效,为该病的治疗方法选择提供参考。方法收集我院2018年1月~2019年12月收治的高龄骨质疏松性桡骨远端骨折患者50例,随机分为对照组(24例)和试验组(26例),对照组行正骨夹板外固定治疗,试验组行手术内固定治疗,比较两组VAS评分、腕关节Gartland-Werley功能评分及并发症发生情况。结果两组治疗后VAS评分低于治疗前,且试验组低于对照组,差异有统计学意义(P<0.05);两组治疗后3个月VAS评分低于治疗后,且试验组低于对照组,差异有统计学意义(P<0.05);试验组腕关节Gartland-Werley功能优良率(92.31%)高于对照组(70.83%),差异有统计学意义(P<0.05);两组均未发生桡神经损伤及皮肤压疮等严重并发症。结论手术内固定治疗高龄骨质疏松性桡骨远端骨折的临床疗效优于正骨夹板外固定,其有助于减轻患者疼痛程度,促进其腕关节功能恢复,安全性良好。

Objective To compare the clinical efficacy of bone splinting external fixation and surgical internal fixation in the treatment of elderly osteoporotic distal radius fractures,and to provide references for the selection of treatment methods for this disease.Methods A collection of 50 elderly patients with osteoporotic distal radius fractures admitted to our hospital from January 2018 to December 2019 were collected.They were randomly divided into a control group(24 cases)and an experimental group(26 cases).The control group received splint external fixation,and the experimental group received surgical internal fixation.The VAS score,wrist Gartland-Werley function score and complications were compared between the two groups.Results The VAS scores of the two groups after treatment were lower than those before treatment,and the experimental group was lower than the control group,the difference was statistically significant(P<0.05);The VAS score of the two groups was lower than that of the treatment group 3 months after treatment,and the experimental group was lower than the control group,the difference was statistically significant(P<0.05);The excellent and good rate of wrist Gartland-Werley function in the experimental group(92.31%)was higher than that of the control group(70.83%),the difference was statistically significant(P<0.05);No serious complications such as radial nerve injury and skin pressure sores occurred in the two groups.Conclusion The clinical effect of surgical internal fixation in the treatment of elderly osteoporotic distal radius fractures is better than that of bone splinting external fixation.It helps to reduce the patient's pain and promote the recovery of wrist joint function.It is safe.
