磁共振弥散加权成像在胰腺良恶性肿块诊断中的应用价值 被引量:3

摘要 目的探讨磁共振弥散加权成像(DWI)在胰腺良恶性肿块诊断中的应用价值。方法收集我院2017年6月~2020年3月收治的64例疑似胰腺病变患者作为研究对象,患者均行DWI诊断,以b超检查为标准,比较不同b值差下(500 s/mm^(2)及1000 s/mm^(2))胰腺正常人群、胰腺良性肿块及恶性肿块患者ADC值以及b值差为1200 s/mm^(2)时胰腺良性肿块及恶性肿块的平均ADC值。结果64例疑似胰腺病变患者中,胰腺正常18例,占28.13%;胰腺良性肿块23例,占35.94%;胰腺恶性肿块23例,占35.94%;b值差为500 s/mm^(2)及1000 s/mm^(2)时,胰腺恶性肿块的ADC值高于良性肿块及胰腺正常人群,胰腺良性肿块的ADC值高于胰腺正常人群,差异有统计学意义(P<0.05);b值差为1200 s/mm^(2)时,胰腺恶性肿块直径较良性肿块长,ADC值高于良性肿块,差异有统计学意义(P<0.05)。结论DWI对于胰腺良恶性肿块的鉴别诊断具有高度的敏感性,有助于胰腺恶性肿块的检出,具有一定临床应用价值。 Objective To explore the application value of MRI diffusion weighted imaging(DWI)in the diagnosis of benign and malignant pancreatic masses.Methods A total of 64 patients with suspected pancreatic disease admitted to our hospital from June 2017 to March 2020 were collected as research objects.All patients were diagnosed by DWI,with b-ultrasound as the standard,compare the ADC values of patients with normal pancreas,benign and malignant pancreatic tumors under different b-value differences(500 s/mm^(2)and 1000 s/mm^(2)),and the average ADC values of pancreatic benign and malignant pancreatic tumors when the b-value difference was 1200 s/mm^(2).Results Among 64 patients with suspected pancreatic disease,18 had normal pancreas,accounting for 28.13%;23 had benign pancreatic mass,accounting for 35.94%;23 cases of pancreatic malignant masses,accounting for 35.94%;When the difference in b value was 500 s/mm^(2)and 1000 s/mm^(2),the ADC value of pancreatic malignant tumors was higher than that of benign tumors and normal pancreas,and the ADC value of benign pancreatic tumors was higher than that of normal pancreas,the difference was statistically significant(P<0.05);When the difference in b value was 1200 s/mm^(2),the diameter of pancreatic malignant tumors was longer than that of benign tumors,and the ADC value was higher than that of benign tumors,the difference was statistically significant(P<0.05).Conclusion DWI is highly sensitive in the differential diagnosis of benign and malignant pancreatic tumors,which is helpful for the detection of malignant pancreatic tumors and has certain clinical application value.
