“医院-社区-家庭”联动的糖尿病延续护理开展现状及存在问题的质性研究

A Qualitative Study on the Current Status and Problems of the Implementation of"Hospital-community-family"Linkage Continuing Care for Diabetes Mellitus

目的了解基于“医院-社区-家庭”联动的糖尿病延续护理的开展现状与存在的问题。方法采用描述性质性研究方法,对上海市浦东新区公利医院12名医院、社区糖尿病管理者进行半结构式访谈,应用主题分析法并借助Nvivo 10.0软件对资料进行整理分析。结果通过访谈共提炼出一级主题2个,分别为“医院-社区-家庭”联动的延续护理服务的开展现状及存在的问题;共提炼出二级主题8个,其中“医院-社区-家庭”联动的延续护理服务的开展现状涉及2个,该服务存在的问题涉及6个。结论应建立分工明确的“医院-社区-家庭”联动的延续护理服务机制,加强多学科合作和护理人员同质化培训,构建简便实用、功能完善的信息化平台,完善相关政策及激励机制,为患者提供全面、专业、可持续的延续护理服务。

Objective To understand the current status and existing problems of continuing diabetes care based on the"hospital-community-family"linkage.Methods A descriptive research method was used to conduct semi-structured interviews with 12 hospitals and community diabetes managers in Gongli Hospital,Pudong New Area,Shanghai.Thematic analysis method was used to collate and analyze the data with the help of Nvivo 10.0 software.Results A total of 2 first-level themes were extracted through the interviews,namely,the current status and existing problems of the continuing care service with"hospital-community-family"linkage;A total of 8 secondary themes were extracted,of which the current development status of the"hospital-community-family"linkage continuing nursing service involved 2 and the problems of the service involved 6 issues.Conclusion It is necessary to establish a“hospital-community-family”continuing care service mechanism with a clear division of labor,strengthen multidisciplinary cooperation and homogeneous training of nursing staff,build a simple,practical,and fully functional information platform,improve relevant policies and incentive mechanisms,and provide patients with comprehensive,professional,and sustainable continuing care services.
