甲状腺癌患者术后凝血功能异常的影响因素分析

Analysis of influencing factors of postoperative coagulation dysfunction in patients with thyroid cancer

目的:探讨甲状腺癌(TC)患者术后凝血功能异常的影响因素。方法:选择某院TC择期手术的患者62例,于术后72 h评估患者凝血功能情况。设计调查问卷表,收集并记录患者相关临床资料,经单因素、多因素分析找出可能导致TC患者术后凝血功能异常的影响因素。结果:62例TC患者术后发生凝血功能异常13例,发生率为20.97%;经单因素、多因素分析,高血脂症、甲状腺全切术、血清钙低表达、皮质醇(Cor)过表达为TC患者术后凝血功能异常的影响因素(OR>1,P<0.05)。结论:TC患者术后有部分患者存在凝血功能异常风险,可能与患者高血脂症、行甲状腺全切术、血清钙低表达、Cor过表达有关,临床可据此拟定针对性的干预方案,对有术后凝血功能异常风险的患者,尽早干预。

Objective:To analyze the influencing factors of postoperative coagulation dysfunction in patients with thyroid cancer(TC).Methods:62 patients with TC who underwent selective surgery in a hospital were included as the research subjects.Coagulation function of the patients was evaluated 72 hours after the operation.A general baseline questionnaire was designed and relevant baseline data of the patients were collected and recorded before finally,through single-and multi-factor analysis,the possible influencing factors that might lead to coagulation dysfunction in patients with TC were found out.Results:Of the 62 patients with TC,13 patients had coagulation dysfunction after surgery,the incidence rate being 20.97%.The single and multi-factor analysis indicated that hyperlipidemia,total thyroidectomy,low expression of serum calcium and over expression of cortisol(Cor)were the influencing factors of postoperative coagulation dysfunction in TC patients(OR>1,P<0.05).Conclusion:Some patients with TC have risk of coagulation dysfunction after operation,which may be related to hyperlipidemia,total thyroidectomy,low serum calcium expression and over expression of Cor.Accordingly,targeted intervention programs should be proposed,and patients with risk of postoperative coagulation dysfunction should be paid attention to and intervened as soon as possible.
