小针刀松解术结合肌肉能量训练技术治疗上交叉综合征的疗效及安全性研究

Clinical Efficacy and Safety of Acupotomy Release Combined with Muscle Energy Training in Treating Upper Cross Syndrome

目的:探究小针刀松解术结合肌肉能量训练技术治疗上交叉综合征临床疗效及安全性。方法:研究共计纳入96例上交叉综合征患者,均由我院2018年6月—2019年10月收治,采取随机数字表法将其分为两组,对照组患者(48例)于取红外线照射结合肌肉能量训练技术治疗,观察组患者(48例)在对照组治疗基础上联合小针刀松解术治疗,比较两组患者临床治疗效果、治疗前后两组患者颈曲值变化、肩胛下角和脊柱棘突间距离变化、颈椎功能障碍指数(NDI)变化、颈部中立位后伸最大肌力变化、头前伸角度(FHA)、圆肩角度(FSA)值变化、治疗前后两组患者疼痛评分(VAS评分)及匹兹堡睡眠指数(PSQI)、汉密尔顿焦虑量表(HAMA)评分变化、治疗不良反应和治疗满意率。结果:观察组患者治疗总有效率(46/48,95.83%)高于对照组(39/48,81.25%),差异有统计学意义(P<0.05);治疗前,两组患者颈曲值、肩胛下角和脊柱棘突间距离、NDI、颈部中立位后伸最大肌力、FHA、FSA、VAS、PSQI及HAMA比较,差异无统计学意义(P>0.05),治疗后各组患者颈曲值、肩胛下角和脊柱棘突间距离、NDI、颈部中立位后伸最大肌力、FHA、FSA、VAS、PSQI及HAMA等指标均改善,观察组患者治疗后颈曲值、肩胛下角和脊柱棘突间距离、NDI、颈部中立位后伸最大肌力、FHA、FSA、VAS、PSQI及HAMA等指标均优于对照组,差异有统计学意义(P<0.05);两组患者治疗过程中均未见不良反应差异无统计学意义(P>0.05),观察组患者治疗满意率(47/48,97.92%)高于对照组(40/48,83.33%),差异有统计学意义(P<0.05)。结论:小针刀松解术结合肌肉能量训练技术治疗上交叉综合征临床疗效显著,患者恢复较好,疼痛缓解,睡眠质量好,心理状态佳,未见不良反应,患者满意率高,值得推广应用。

Objective:To explore the clinical efficacy and safety of acupotomy release combined with muscle energy training in the treatment of upper cross syndrome.Methods:96 cases of upper cross syndrome were randomly divided into the control group(n=48)and the observation(n=48).The control group was treated with infrared irradiation combined with muscle strength training technology;on which basis,the observation group was also treated with acupotomy release.The curative effect,the changes of neck curvature,distance of angulus inferior scapulae to spinous process,NDI,maximum neck muscle strength,FHA and FSA,the scores of VAS,PSQI and HAMA,as well as adverse reactions and satisfactory rate were observed and compared between the two groups.Results:The total effective rate of the observation group was 95.83%(46/48),which was significantly higher than 81.25%(39/48)of the control group(P<0.05).There were no statistical differences in neck curvature,distance of angulus inferior scapulae to spinous process,NDI,maximum neck muscle strength,FHA and FSA,as well as the scores of VAS,PSQI and HAMA between the two groups before the treatment(P>0.05);which were all improved after the treatment compared to those before the treatment in the two groups(P<0.05),and the improvements in the observation group were more significant(P<0.05).No adverse reactions were found during the treatment in the two groups,and there was no statistical difference between the two groups(P>0.05).The satisfaction rate of the observation group was 97.92%(47/48),which was significantly higher than 83.33%(40/48)of the control group(P<0.05).Conclusion:Acupotomy release combined with muscle energy training techniques has a remarkable clinical effect on the treatment of upper cross syndrome,with good recovery,pain relief,good sleep quality,good psychological state,no adverse reactions and a high satisfaction rate,which is worthy of promotion and application.
