社区慢性病管理中应用多学科联动模式进行管理的效果研究

Research on the Effect of Applying Multi-disciplinary Linkage Model in Community Chronic Disease Management

目的探究社区慢性病管理中应用多学科联动模式进行管理的效果。方法选取2019年1月—2020年1月在社区建档的慢性病患者100例,随机分为对照组与观察组,各50例。对照组应用常规的社区慢性病管理模式,观察组应用多学科联动模式进行管理,观察两组患者的慢性病管理效果。结果管理前,两组患者的高血压控制、糖尿病控制及护理依从性情况等对比差异无统计学意义(P>0.05);管理后,两组患者的高血压控制、护理依从性及糖尿病控制改善情况等对比差异有统计学意义(P<0.05)。结论社区慢性病管理中应用多学科联动模式效果显著,能有效提高患者的生活质量及自我管理能力,值得临床推广。

Objective To explore the effect of applying the multidisciplinary linkage model in the management of chronic diseases in the community.Methods A total of 100 chronic disease patients who were filed in the community from January 2019 to January 2020 were selected and randomly divided into a control group and an observation group,with 50 cases in each group.The control group applied the conventional community chronic disease management model,and the observation group applied the multidisciplinary linkage model for management to observe the chronic disease management effect of the two groups of patients.Results Before management,there was no statistically significant difference in the,the hypertension control,diabetes control,and nursing compliance of the two groups of patients(P>0.05);after management,the hypertension control,nursing compliance,and diabetes control improvement of the two groups were compared,the difference was statistically significant(P<0.05).Conclusion The application of the multidisciplinary linkage model in the management of chronic diseases in the community has a significant effect,which can effectively improve the quality of life and self-management ability of patients,and is worthy of clinical promotion.
