成人咽喉反流性疾病对咽鼓管功能的影响研究

Effect of laryngopharyngeal reflux disease on eustachian tube function in adults

目的探讨成人咽喉反流性疾病所致的咽喉反流(LPR)对咽鼓管功能的不良影响及其程度。方法本研究分为观察组和对照组,观察组为经反流症状指数(RSI)量表及反流体征(RFS)量表筛查评估后诊断为LPR且病程>3个月的成人患者;对照组为参加门诊体检的健康成人。采用咽鼓管测压(TMM)和咽鼓管功能障碍症状评分量表(ETDQ-7)评估2组观察对象的咽鼓管功能。结果2组比较,ETDQ-7各症状评分差异均有统计学意义(P<0.05)。TMM结果显示,在不同压力下(30、40、50 mbar),对照组和观察组的咽鼓管异常开放比例差异均有统计学意义(P<0.05)。结论成人咽喉反流性疾病所致的LPR与咽鼓管功能障碍有关,LPR可能是引起部分咽鼓管功能障碍的重要病因之一。

Objective To investigate the adverse effects of laryngopharyngeal reflux(LPR)on the eustachian tube function in adults with laryngopharyngeal reflux disease.Methods There are observation group and the control group in the study.The observation group were adult patients diagnosed as LPR after reflux symptom index(RSI)and reflux finding score(RFS)scale assessment and with the case course more than 3 months.The control group were healthy adults who participated in outpatient physical examination.Tubomanometry(TMM)and the seven-item eustachian tube dysfunction questionnaire(ETDQ-7)were used to measure the tubal function.Results There were statistically significant differences in the scores of seven eustachian tube dysfunction symptoms(P<0.05).TMM test results showed statistically significant differences in abnormal opening of eustachian tubes between the control group and the observation group under different pressures(30,4,50 mbar;P<0.05).Conclusions LPR caused by adult pharyngeal reflux disease was related to eustachian tube dysfunction.LPR may be an important cause of partial eustachian tube dysfunction.
