摘要
目的探讨22 MHz超高频超声用于评估先天性耳前瘘管的准确性。方法应用22 MHz超声探头采集33例(34耳)先天性耳前瘘管患者的二维及彩色血流图像,分析超声图像特征,与术中诊断及病理诊断金标准进行比较。结果34耳中有分支者共计18耳,没有分支者共计16耳。瘘管结构距体表深度为2.0~7.2 mm,耳前瘘管长度为3.0~15.7 mm。经手术证实对内口盲端的显示率为85%(29/34)。分支走行准确率与病理结果一致性较高(Kappa=0.761,P<0.05)。部分术前瘢痕及感染性病灶对瘘管显示有一定影响。结论22 MHz超高频超声可提供清晰的二维图像及血流图像,能较准确地评估先天性耳前瘘管的综合病情。
Objective To investigate the efficacy of 22 MHz ultra-high-frequency ultrasound in assessing the clinical condition of congenital preauricular fistula.Methods The two-dimensional ultrasound images and color Doppler flow images in the same section of 33 congenital preauricular fistula patients(34 ears)were collected with 22 MHz ultrasound.According to whether the fistula could be recognized,these ultrasound images were classified and compared.Results Ultra-high-frequency ultrasound was superior in distinguishing the branches of fistula and scar tissue.There were eighteen ears which have branches,16 ears which has no branches.The depth of fistula structure from the body surface was 2.0~7.2 mm and the length of the preauricular fistula was 3.0~15.7 mm.It was confirmed by surgery that the rate of internal oral blindness was displayed 85%(29/34).The accuracy of direction of the branches was consistent with the pathological results(Kappa=0.761,P<0.01).Part of the preoperative scars and infectious lesions had a certain impact on the display of fistula.Conclusions Ultra-high-frequency ultrasound can provide clearer two-dimensional ultrasound images and color Doppler flow images about congenital preauricular fistula.The comprehensive condition of congenital preauricular fistula can be evaluated more accurately.
