摘要
目的探讨全身成像三维量化(GI3DQ)评价左心室收缩功能的可行性。方法选取45例患者,采用二维Simpson′s法(以下简称2D法)测量左心室舒张末期容积(EDV)和收缩末期容积(ESV),每搏输出量(SV),得出左心室射血分数(LVEF);用实时三维超声心动图(RT-3DE)采集心尖四腔心全容积图像,导入QLABadv,测量EDV和ESV,得出LVEF;将实时三维心尖四腔心全容积图像导入GI3DQ三维图像工作站脱机测量EDV和ESV,得出LVEF。结果GI3DQ与2D法评价左心室功能参数测值,EDV、ESV、SV、LVEF比较差异均无统计学意义(P>0.05),二者评价LVEF相关性较好(r=0.870),两种方法进行一致性分析差异无统计学意义(P>0.05);GI3DQ与RT-3DE评价左心室功能参数测值,EDV、SV比较差异均无统计学意义(P>0.05),但二者ESV、LVEF比较差异有统计学意义(P<0.05),二者评价LVEF值相关性较好(r=0.920),两种方法进行一致性分析差异无统计学意义(P>0.05)。结论GI3DQ可对左心室收缩功能进行准确的定量评估,为临床评价左心室收缩功能提供了更加准确、简便的方法。
Objective To evaluate the feasibility of general imaging 3-dimensional quantification(GI3DQ)in assessing the left ventricular systolic function.Methods Forty-five patients were selected.The left ventricular end diastolic volume(EDV),end systolic volume(ESV)and stroke volume(SV)were measured by adopting the dimensional Simpson′s method(2D method),and the left ventricular ejection fraction(LVEF)was obtained;the full volume image of apical four chamber heart was collected by using the real-time three-dimensional echocardiography(RT-3DE),by importing QLABadv,EDV and ESV were measured to obtain LVEF;the full volume image of three-dimensional apical four chamber heart was imported into the GI3DQ three-dimensional imaging work station for offline measuring EDV and ESV,and LVEF was obtained.Results The measured values of parameters EDV,ESV,SV and LVEF for evaluating the left ventricular function had no statistically significant difference between the GI3DQ and 2D method(P>0.05),the correlation of both in evaluating LVEF was good(r=0.870),the difference in the consistency analysis had no statistical difference between the two methods(P>0.05);in evaluating the measured values of left ventricular function parameters by GI3DQ and RT-3DE,EDV and SV had no statistically significant difference(P<0.05),but ESV and EF had statistically significant difference betweeen them(P<0.05).The both for evaluating the LVFE value had good correlation(r=0.920),and the difference in the consistency analysis had no statistical significant between the two methods(P>0.05).Conclusion GI3DQ can accurately and quantitatively evaluate the left ventricular systolic function,which provides a more accurate and simple method for clinical evaluation of left ventricular systolic function.
