双动全髋关节置换术与半髋关节置换术治疗老年股骨颈骨折近期效果对比

Comparison of the short-term effects of double-action total hip replacement versus hemi-hip replacement in the treatment of elderly femoral neck fractures

目的比较双动全髋关节置换术与半髋关节置换术治疗老年股骨颈骨折的近期效果。方法抽取2018年1月至2020年7月晋中市第一人民医院收治的老年股骨颈骨折患者120例,年龄61~84岁,依照治疗方案分为两组,每组60例。研究组行双动全髋关节置换术,对照组行半髋关节置换术。回顾性对照分析两组手术指标、术后并发症及髋关节功能。结果两组手术操作时间、术中出血量比较,差异未见统计学意义(P>0.05)。研究组术后首次下床活动时间、术后住院时间少于对照组(P<0.05)。研究组术后并发症发生率为5.00%(3/60),小于对照组的23.33%(14/60),P<0.05。两组术前Harris量表评分比较,P>0.05;术后6个月,研究组Harris量表评分为(90.36±5.4)分,高于对照组的(81.28±6.03)分,P<0.05。结论与半髋关节置换术比较,双动全髋关节置换术治疗老年股骨颈骨折近期疗效显著,有利于促进其髋关节功能尽快恢复和减少术后并发症。

Objective To compare the short-term effects of double-action total hip arthroplasty and hemi-hip replacement in the treatment of elderly femoral neck fractures.Methods A total of 120 elderly patients with femoral neck fractures,aged 61-84 years,who were admitted to the Jinzhong First People’s Hospital from January 2018 to July 2020,they were divided into two groups according to the treatment plan,with 60 cases in each group.The study group underwent double-action total hip arthroplasty,and the control group underwent half hip arthroplasty.The surgical indicators,postoperative complications and hip function of the two groups were compared and analyzed retrospectively.Results There was no significant difference in operation time and intraoperative blood loss between the two groups(P>0.05).The first time of out of bed after operation and the postoperative hospitalization time of the study group were less than those of the control group(P<0.05).The incidence of postoperative complications in the study group was 5.00%(3/60),which was less than 23.33%(14/60)in the control group,P<0.05.The harris scale scores were comparison between the two groups before operation,(P>0.05);6 months after operation,the harris scale score of the study group was(90.36±5.4)points,which was higher than(81.28±6.03)points of the control group,(P<0.05).Conclusions Compared with hemi-hip replacement,double-action total hip arthroplasty in the treatment of elderly patients with femoral neck fracture has a significant short-term effect,which is beneficial to promote the rapid recovery of hip joint function and reduce postoperative complications.
