超声定量评估髋关节前上盂唇面积及关节囊厚度的可行性研究 被引量:2

Feasibility of the Ultrasonographic Assessment of the Labral Cross-sectional Area of Anterosuperior Acetabular Labrum and the Thickness of Anterior Articular Capsule

摘要 目的:探讨超声测量无症状人群髋关节前上盂唇截面积(CSA)、前关节囊厚度测量重复性。方法:测量40位(80髋)无髋关节症状患者髋关节前上盂唇CSA及前关节囊厚度。男20人,女20人,年龄21~49岁,平均32.4±8.2岁。2位超声医师分别测量髋关节前上盂唇CSA、前关节囊厚度。采用组内相关系数(ICC)评价测量重复性。结果:男性组盂唇CSA为0.259±0.067 cm^(2),女性组盂唇CSA为0.253±0.045 cm^(2)。男性关节囊厚度为0.564±0.119 cm,女性关节囊厚度为0.539±0.101 cm。男性组、女性组和左髋、右髋盂唇CSA、关节囊厚度差异均无统计学意义(P<0.05)。盂唇CSA重复测量ICC为0.855,测量者间ICC为0.688。关节囊厚度重复测量ICC为0.904,测量者间ICC为0.827。结论:盂唇CSA测量重复性为好至非常好,关节囊厚度测量重复性为非常好。 Objective To explore the repeatability of ultrasonic measurement of the labral cross-sectional area(CSA) of the anterosuperior acetabular labrum and the thickness of the anterior articular capsule. Methods A total of 80 hips of 40 asymptomatic patients(20 males and 20 females,ranging in age from 21 to 49 with an average age of 32.4 ± 8.2) were included in this study. Two ultrasound doctors measured their CSA of the anterosuperior acetabular labrum and the thickness of the anterior articular capsule independently. The intraclass correlation coefficient(ICC) was used to compare the consistency of their measurements. Results The average labral CSA of the male and female group was 0.259 ± 0.067 cm^(2) and 0.253 ± 0.045 cm^(2) respectively. The thickness of anterior articular capsule of the male and female group was 0.564 ± 0.119 cm and 0.539 ± 0.101 cm respectively. There was no significant difference in the average CSA and thickness of the articular capsule between different sexes and between the left and right hips(P<0.05). The ICC of repeated measuring and different doctors of CSA was 0.855 and 0.688 respectively,while that of the thickness of the anterior articular capsule was 0.904 and 0.827 respectively. Conclusion The measurement of CSA in the anterosuperior acetabular labrum and the thickness of the anterior articular capsule have good and even perfect consistency between different ultrasound doctors.
