摘要
目的:研究血清半胱氨酸(Cysteine,Cys)和新生儿神经行为测定(Neonatal Neurobehavioral Assessment,NANB)评分预测新生儿缺血缺氧性脑病(Hypoxic-ischemic encephalopathy,HIE)的价值。方法:选取我院2018-01~2019-01收治的60例缺血缺氧性脑病患儿为研究对象,另选我院同期健康新生儿50例。统计两组儿童基础资料,包括性别、胎龄、日龄、体重、收缩压(Systolic blood pressure,SBP)、舒张压(Diastolic blood pressure,DBP)、谷丙转氨酶(Alanine aminotransferase,ALT)、谷草转氨酶(Aspartate transaminase,AST)、血尿素氮(Blood urea nitrogen,BUN)、血液肌酐(Creatinine,Cr)、Cys及NANB评分。绘制ROC曲线分析Cys、NANB评估HIE的价值,最后经Logistic回归性分析Cys、NANB是否为HIE的危险因素。结果:两组新生儿性别、胎龄、日龄、体重、SBP、DBP、ALT、AST、BUN、Cr对比差异无统计学意义(P>0.05);HIE患儿组Cys水平显著高于健康新生儿组,NANB评分显著低于健康新生儿组,差异有统计学意义(P<0.05)。经ROC分析Cys、NANB的下曲线面积分别为0.959、0.911。通过Logistic回归性分析证明Cys>1.605、NANB评分≤35.390是HIE的危险因素。结论:HIE与Cys、NANB有一定相关性,Cys、NANB可成为早期评估新生儿HIE的重要指标。
Objective:To investigate the value of serum cysteine(Cys)and neonatal neurobehavioral measurement(NaNb)scores in predicting neonatal hypoxic-ischemic encephalopathy(HIE).Methods:From January 2018 to January 2019,60 children with hypoxic ischemic encephalopathy and 50 healthy newborns in the same period were selected as subjects.Basic data of children in two groups were analyzed,including gender,gestational age,age of day,body weight,systolic blood pressure(SBP),diastolic blood pressure(DBP),alanine aminotransferase(ALT),aspartate aminotransferase(AST),blood urea nitrogen(BUN),blood creatinine(Cr),Cys and NaNb score.ROC curves were drawn to analyze Cys and NaNb to evaluate the value of HIE.Logistic regression analysis was performed to determine whether Cys and NaNb were risk factors for HIE.Results:There were no significant differences in newborn sex,gestational age,day age,body weight,SBP,DBP,ALT,AST,BUN and Cr between the two groups(P>0.05).The level of Cys in HIE group was significantly higher than that in healthy newborn group,and the score of NaNb was significantly lower than that in healthy newborn group,with statistical significance(P<0.05).ROC analysis showed that the area of the lower curve of Cys and NaNb was 0.959 and 0.911,respectively.Logistic regression analysis showed that Cys>1.605 and NaNb score≤35.390 were risk factors for HIE.Conclusion:HIE is correlated with Cys and NaNb to some extent,and good Cys and NaNb can be an important indicator for early evaluation of neonatal HIE.
