Risk assessment of nosocomial infection based on Logistic regression analysis

摘要 目的探讨Logistic回归分析的医院感染风险评估。方法选取某院2018年上报的200例医院感染患者作为研究对象,采用病历调查与床旁调查相结合的方法进行医院感染现患率调查,对危险因素进行Logistic回归分析。结果7352例在院患者中,发生医院感染200例(211例次),医院感染率2.72%,医院例次感染率2.87%,感染部位以上呼吸道为主,肿瘤内科医院例次感染率最高,科室间医院例次感染率比较差异有统计学意义(P<0.05)。经χ^(2)检验单独计算性别、年龄、住院天数、合并慢性病、化疗、抗菌药物应用、使用免疫抑制药、入住ICU、有创性操作、手术时间等与医院感染的关系,差异有统计学意义(P<0.05),对这10项指标进行Logistic回归分析发现,均为医院感染的独立危险因素。结论医院感染与诸多因素有关,应该制订相应控制措施,例如合理使用抗菌药物、严格无菌操作、定期进行环境卫生学监测、加强药物和饮食指导等。 Objective To explore the risk assessment of nosocomial infection based on Logistic regression analysis.Methods A total of 200 patients with nosocomial infection reported in our hospital in 2018 were selected as the research objects.A combination method of medical record investigation and bedside investigation was used to study the prevalence rate of nosocomial infection,and Logistic regression analysis was performed on risk factors.Results A total of 7352 inpatients were investigated,and there were 200 patients with nosocomial infection and 211 cases,with the rate of nosocomial infection 2.72%and the case rate of nosocomial infection 2.87%.The infection site was mainly the upper respiratory tract,and the oncology department had the highest case rate of nosocomial infection.There was a statistically significant difference in the case rate of nosocomial infection between departments(P<0.05).χ^(2)test was used to calculate the relationship between gender,age,length of hospitalization,combined chronic disease,chemotherapy,antimicrobial application,use of immunosuppressive agents,admission to ICU,invasive operation,operation time and nosocomial infection,and the difference was statistically significant(P<0.05).Logistic regression analysis showed that these ten factors were all independent risk factors for nosocomial infection.Conclusion Nosocomial infection is related to many factors.It is necessary to formulate corresponding control measures,such as rational use of antibiotics,strict aseptic operation,regular environmental hygiene monitoring,strengthening drug and diet guidance and so on.
