对比不同入路途径对胫骨平台后外侧骨折的治疗效果

Comparison of Therapeutic Effects of Different Approaches on Posterolateral Tibial Plateau Fractures

目的探讨经前外侧入路内固定手术同常规后外侧入路内固定手术方法在胫骨平台后外侧骨折临床治疗中的应用效果。方法选取2019年1月-2020年3月于我院住院拟行手术治疗的胫骨平台后外侧骨折患者共78例,随机分成观察组及对照组,每组各39例。观察组患者运用改良后的前外侧入路内固定法,对照组运用常规后外侧入路内固定法进行治疗。于治疗过程中对比观察两组患者切口长度、手术时长、骨折愈合时间、术中出血量、手术切口愈合时间、膝关节屈曲活动度及HSS评分等指标。结果经不同手术方法治疗后,观察组在切口长度、术中出血量、手术总时间及手术切口愈合时间等指标比较中,要优于对照组患者(P<0.05);观察组患者膝关节屈曲活动度及HSS评分大于对照组患者,差异具有统计学意义(P<0.05)。结论前外侧入路内固定手术相较于常规后外侧入路内固定手术方法在胫骨平台后外侧骨折临床治疗中的应用效果良好。

Objective To investigate the clinical effect of anterior lateral approach internal fixation surgery and conventional posterolateral approach internal fixation surgery in the treatment of posterolateral tibial plateau fractures.Methods Selected 78 patients with posterior lateral tibial plateau fractures from January 2019 to March 2020,divided into observation group and control group.39 cases in each group.Observation group were treated with the modified anterolateral approach internal fixation method,control group was treated with the conventional posterolateral approach internal fixation method.Compared the indexes of incision length,operation time,fracture healing time,intraoperative blood loss,surgical incision healing time,knee flexion activity and HSS score.Results After different surgical methods,the observation group was better than the control group in incision length,intraoperative blood loss,total operation time and incision healing time(P<0.05);The flexion range of motion and HSS score of the observation group were higher than those of the control group,the difference was statistically significant(P<0.05).Conclusion Anterior lateral approach internal fixation surgery is more effective than conventional posterolateral approach internal fixation surgery in the tibia.
