膀胱压力容量评定系统在脊髓损伤神经源性膀胱管理中的应用效果 被引量:3

摘要 目的探讨膀胱压力容量评定系统在脊髓损伤神经源性膀胱管理中的应用效果。方法选取2018年10月至2019年5月该院康复医学科收治的50例因脊髓损伤引起的神经源性膀胱患者。采用膀胱压力容量评定系统评定膀胱功能。根据监测的膀胱容量、膀胱压力及逼尿肌、外括约肌的状况制定相关的治疗、康复及护理措施。干预4周后,评估治疗前后患者膀胱功能的疗效和预后。结果治疗前后患者的膀胱安全容量、最大膀胱压力及残余尿量、膀胱功能疗效及预后比较,差异均有统计学意义(P<0.05)。35例患者能够自主排尿且残余尿量低于100 mL,11例患者需要间歇导尿,4例患者需要留置尿管。结论膀胱压力容量评定系统能有效判断神经源性膀胱患者的膀胱压力-容量关系,采取针对性的膀胱功能管理方案,能有效恢复膀胱功能,提高患者康复效果。 Objective To investigate the application effect of bladder pressure capacity assessment system in neurogenic bladder management of spinal cord injury.Methods A total of 50 patients with neurogenic bladder caused by spinal cord injury were selected from October 2018 to March 2019 in the department of rehabilitation medicine of our hospital.Bladder pressure capacity assessment system was used to evaluate bladder function.According to the monitored bladder capacity,bladder pressure,detrusor and external sphincter status,the relevant treatment,rehabilitation and nursing measures were developed.After 4 weeks of intervention,the efficacy and prognosis of bladder function before and after treatment were evaluated.Results Before and after treatment,the bladder safety capacity,maximum bladder pressure,residual urine volume,and bladder function efficacy and prognosis were statistically significant(P<0.05).Thirty-five patients were able to urinate autonomously and the residual urine volume was less than 100 mL,eleven patients required intermittent catheterization,and four patients required indwelling catheter.Conclusion The bladder pressure capacity assessment system can effectively determine the relationship between bladder pressure and volume in patients with neurogenic bladder,and adopt a targeted bladder function management scheme to effectively restore bladder function and improve the rehabilitation effect of patients.
