小剂量胺碘酮联合美托洛尔治疗小儿心律失常的疗效及对miR-1、CK-MB、cTnI的影响 被引量:5

摘要 目的探究小剂量胺碘酮联合美托洛尔治疗小儿心律失常的疗效及对miR-1、肌酸激酶同工酶(CK-MB)、肌钙蛋白I(cTnI)的影响。方法选取2018年3月至2019年8月该院收治的心律失常患儿95例,采用随机数字表法分为观察组(48例)与对照组(47例)。对照组给予小剂量胺碘酮治疗,观察组给予小剂量胺碘酮联合美托洛尔治疗,对比2组临床疗效、miR-1、CK-MB、cTnI水平及心率变异性指标。结果观察组总有效率高于对照组,差异均有统计学意义(P<0.05);治疗后,观察组血清miR-1、CK-MB、cTnI水平均低于对照组,差异有统计学意义(P<0.05);治疗后观察组心率变异性各指标RR间期总体标准差(SDNN)、相邻RR间期差值的均方根(RMSSD)、RR间期平均值的标准差(SDANN)、相邻间期差值50 ms占总RR间期数的百分比(PNN50)高于对照组,差异均有统计学意义(P<0.05)。结论小剂量胺碘酮联合美托洛尔可促进心律失常患儿心律转复,改善心率变异性指标,降低血清miR-1、CK-MB、cTnI水平,具有明显的心肌保护作用。 Objective To explore the effect of low-dose amiodarone combined with metoprolol in the treatment of infantile arrhythmia and its influences on miR-1,creatine phosphokinase isoenzyme(CK-MB)and cardial troponin I(cTnI).Methods Ninty-five patients with arrhythmia were selected in the hospital from March 2018 to August 2019 were randomly divided into the observation group(48 cases)and the control group(47 cases).The control group was treated with low-dose amiodarone,and the observation group was treated with low-dose amiodarone combined with metoprolol.The clinical efficacy,miR-1,CK-MB,cTnI levels and heart rate variability indexes were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05);after treatment,the serum levels of miR-1,CK-MB and cTnI in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05);after treatment,the SDNN,RMSSD,SDANN,PNN50 in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion Low-dose amiodarone combined with metoprolol can promote heart rate reversion,improve heart rate variability index,and reduce serum miR-1,CK-MB,cTnI levels in children with arrhythmia,and has obvious cardioprotective effects.
