术中电生理监测对原发性面肌痉挛MVD预后的影响 被引量:5

摘要 目的探讨术中电生理监测对原发性面肌痉挛(primary hemifacial spasm,pHFS)预后的影响。方法回顾性分析60例行微血管减压术(microvascular decompression,MVD)pHFS病人的临床资料,33例术中行电生理监测作为研究组,27例未行电生理监测作为对照组。对两组术后疗效、并发症、复发情况等进行统计学分析。结果研究组术前均可引出侧方扩散反应,术中消失28例,波幅明显下降3例,较术前无变化2例;脑干听觉诱发电位监测正常26例,异常7例;经头皮颅骨刺激面肌诱发电位监测正常30例,异常3例。术后1个月两组有效率差异无统计学意义(P> 0.05),而术后1年两组有效率差异有统计学意义(P <0.05);研究组1个月及1年后并发症发生率与复发情况明显低于对照组(P <0.05)。结论 MVD术中应用电生理监测技术有实时预警作用,可提高手术有效率,降低术后并发症及复发率。
