摘要 从物理的力学角度来考虑腹壁疝的形成及发展,腹壁受各方向力的作用下,通过自身接触性的张力,维持腹壁的稳定性。但在加速度、净力不为0的情况下,根据拉普拉斯定律及帕斯卡原理,即同样的腹内压下,疝囊、缺损部分承受的张力要明显大于正常腹壁,此时张力会造成腹壁疝的发生、发展。根据力学原理,腹壁疝手术考量应该尽量恢复疝囊缺损部位张力可承受性(即缺损的关闭),还需要保持张力加速度及静力为0(即立体缝合下的疝囊折叠、齿轮效应),这样才能妥善治疗腹壁疝并最大程度减少疝的复发。 The paper considers the formation and development of ventral hernia from the point of view of physical engineering.Affected by the force from all direction,the abdominal wall keeps stability via its own tension.However,due to acceleration of tension,according to Laplace’s law and Pascal principle,that is,under the same abdominal pressure,the tension of the sac or defect of the abdominal wall is significantly larger than the normal part,at this time ventral hernia occurs and development.According to the mechanical principle,the surgical consideration to repair of ventral hernia should try to restore the strain tolerance of the defect site of the sac(i.e.,the closing of the defect),but also need to release the acceleration of tension(i.e.,threedimensional suture of the sac folding,gear effect),in order to properly treat the ventral hernia,and minimize its recurrence.
