摘要
目的分析小儿感染性肺炎诊断中血清降钙素原(PCT)与C反应蛋白(CRP)联合检测的效果及临床价值。方法在2019年3月至2020年4月到我院治疗的确诊感染性肺炎患儿中,选取100例作为本次的研究对象,将其根据病原学检查及血清学检测结果分为了细菌感染组、病毒感染组和肺炎支原体感染组,分别为30例、30例、40例,并选取同时期到我院体检进行了PCT、CRP检测的健康小儿50例作为本次研究的对照组。对比各组患儿的PCT、CRP指标,并对PCT、CRP单独检测与联合检测时的诊断结果进行对比。结果细菌感染组的PCT、CRP均显著高于病毒感染组、肺炎支原体感染组和对照组(P<0.05),病毒感染组、肺炎支原体感染组的PCT、CRP稍高于对照组,但差异无统计学意义(P>0.05)。两个指标联合进行检测时,检测的灵敏度、特异度、阳性预测值、阴性预测值均得到提高。结论PCT与CRP联合检测能够有效鉴别细菌性肺炎和其他病原体引起的肺炎,有利于患儿得到早期的抗生素治疗,避免病情进一步恶化,对小儿感染性肺炎诊断具有重要的临床价值。
Objective To analyze the effect and clinical value of combined detection of serum procalcitonin(PCT)and C-reactive protein(CRP)in the diagnosis of infectious pneumonia in children.Methods From March 2019 to April 2020,100 children with confirmed infectious pneumonia who were treated in our hospital were selected as the research objects.According to the results of etiological and serological tests,they were divided into bacterial infection group,viral infection group and Mycoplasma pneumoniae infection group,which were 30,30 and 40 respectively.50 healthy children who had PCT and CRP tests in our hospital at the same time were selected Cases as the control group of this study.The PCT and CRP indicators of the children in each group were compared,and the diagnostic results of the PCT and CRP single and combined tests were compared.Results PCT and CRP in bacterial infection group were significantly higher than those in virus infection group,Mycoplasma pneumoniae infection group and control group(P<0.05).PCT and CRP in virus infection group and Mycoplasma pneumoniae infection group were slightly higher than those in control group,but the difference was not statistically significant(P>0.05).The sensitivity,specificity,positive predictive value and negative predictive value of the two indexes were improved.Conclusion PCT and CRP combined detection can effectively identify bacterial pneumonia and pneumonia caused by other pathogens,which is conducive to the early antibiotic treatment of children,to avoid further deterioration of the disease,and has important clinical value for the diagnosis of infectious pneumonia in children.
