改良腹腔镜下子宫悬吊术联合圆韧带缩短术治疗子宫脱垂的临床效果及其安全性

Clinical effect and safety of modified laparoscopic uterine suspension combined with round ligament shortening in the treatment of uterine prolapse

目的探讨改良腹腔镜下子宫悬吊术联合圆韧带缩短术治疗子宫脱垂的临床效果及其安全性。方法选取郴州市第四人民医院2019年9月—2020年9月收治的子宫脱垂患者86例,依据不同治疗方法分为参照组与联合组,各43例。参照组行阴式子宫切除术治疗,联合组行改良腹腔镜下子宫悬吊术联合圆韧带缩短术治疗。比较2组临床疗效、术中出血量、手术时间、住院时间,手术前后生活满意度量表(SWLS)评分,并比较2组并发症发生情况。结果联合组总有效率高于参照组,术中出血量低于参照组,手术时间、住院时间短于参照组(P<0.05)。术前2组SWLS评分比较,差异无统计学意义(P>0.05);术后3个月联合组SWLS评分高于参照组(P<0.05)。联合组并发症发生率低于参照组(P<0.05)。结论改良腹腔镜下子宫悬吊术联合圆韧带缩短术治疗子宫脱垂的临床效果良好,可明显改善患者术后指标,提升生活满意度,且安全性较高。

Objective To investigate the clinical effect and safety of modified laparoscopic uterine suspension combined with round ligament shortening in the treatment of uterine prolapse.Methods A total of 86 cases of patients with uterine prolapse were selected from September 2019 to September 2020 in Chenzhou Fourth People’s Hospital,which were randomly divided into reference group and combined group according to different treatment methods,43 cases in each group.The control group was treated with vaginal hysterectomy,and the combined group was treated with modified laparoscopic uterine suspension combined with round ligament shortening.The clinical efficacy,intraoperative blood loss,operation time,hospital stay,life satisfaction scale(SWLS)score before and after operation,and complications of the two groups were compared.Results The total effective rate of the combined group was higher than that of the reference group,the intraoperative blood loss was lower than that of the reference group,and the operation time and hospital stay of the combined group were shorter than those of the reference group(P<0.05).There was no significant difference in SWLS score between the two groups before operation(P>0.05);the SWLS score of the combined group was higher than that of the control group 3 months after operation(P<0.05).The incidence of complications in the combined group was lower than that in the control group(P<0.05).Conclusion The clinical effect of improved laparoscopic uterine suspension combined with round ligament shortening in the treatment of uterine prolapse is good,which can significantly improve the postoperative indicators of patients,improve life satisfaction,and with high safety.
