右归丸结合温针灸治疗脾肾阳虚型股骨头坏死患者的临床效果

Clinical Effect of Yougui Pills Combined with Warm Needle Moxibustion on Patients with Femoral Head Necrosis of Spleen and Kidney Yang Deficiency

目的探讨右归丸与温针灸联合治疗脾肾阳虚型股骨头坏死患者的临床效果。方法选取2018年6月至2019年8月在沈阳市骨科医院确诊为股骨头坏死的90例患者作为研究对象,依据随机数字表法分为对照组和观察组,各45例。对照组给予口服右归丸治疗,观察组在对照组基础上采用温针灸治疗,比较两组临床疗效、中医证候评分、视觉模拟评分法(VAS)评分、髋关节功能评分及白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平变化。结果观察组治疗有效率为95.56%,明显高于对照组的82.33%(P<0.05);治疗后,两组患者中医证候积分明显降低,且观察组明显低于对照组,差异有统计学意义(P<0.05);治疗后,两组患者VAS评分降低,且观察组患者VAS评分低于对照组,差异有统计学意义(P<0.05);治疗后,两组患者Harris评分升高,且观察组Harris评分高于对照组,差异有统计学意义(P<0.05);治疗后,两组患者IL-1β、IL-6、TNF-α水平降低,且观察组IL-1β、IL-6、TNF-α水平低于对照组,差异有统计学意义(P<0.05)。结论右归丸结合温针灸能够提高脾肾阳虚型股骨头坏死的髋关节功能,有效降低炎症介质水平。

Objective To explore the clinical effect of Yougui Pills combined with warm needle moxibustion in the treatment of patients with femoral head necrosis of spleen and kidney yang deficiency. Methods A total of 90 patients with femoral head necrosis diagnosed as spleen and kidney yang deficiency admitted to Shenyang Orthopedic Hospital were selected as the research objects and were randomly divided into a control group and a treatment group, with 45 cases in each group. The control group was given oral Yougui Pill. The observation group was treated with warm needling moxibustion plus Yougui Pill. Clinical efficacy,TCM syndrome score, visual pain simulation score(VAS), hip joint function score, changes in levels of interleukin-1β(IL-1β), interleukin-6(IL-6), and tumor necrosis factor-α(TNF-α) were compared. Results The effective rate of treatment in the observation group was 95.56%, which was significantly higher than 82.33% in the control group(P<0.05);after treatment, the TCM syndrome scores of the 2 groups were significantly reduced, and the observation group was significantly lower than the control group, the difference was statistically significant(P<0.05);after treatment, the VAS score of the 2 groups of patients decreased, and the VAS score of the observation group was lower than that of the control group, the difference was statistically significant(P<0.05);after treatment, the Harris score of the 2 groups increased, and the Harris score of the observation group was higher than that of the control group, and the difference was statistically significant(P<0.05);after treatment, the levels of IL-1β, IL-6, and TNF-α in the 2 groups decreased, and IL-1β, IL-6 The level of TNF-α of the observation group was lower than that of the control group, and the difference was statistically significant(P<0.05). Conclusion Yougui Pill combined with warm needling moxibustion can improve hip joint function of femoral head necrosis of spleen and kidney yang deficiency type, and effectively reduce the level of inflammation.
