Visit methods and triage assessments of emergency patients in psychiatric hospital

目的探究精神病专科医院急诊来院方式及分诊评估情况。方法选取2017年4月至2017年11月于广州市某三甲精神专科医院急诊就诊的患者1585例,采用自编调查表收集患者的临床资料、来院方式、诊治情况等信息。比较不同来院方式。结果1585例患者中,就诊方式中患者自己来、家属送来、救护车送来、警察送来占比分别为5.9%、80.9%、3.1%、10.1%。主要诊断为精神分裂症(368例,23.2%)、双相障碍(357例,22.5%)、抑郁症(229例,14.4%)。病情严重程度分级情况中急危、急重、急症、亚急症患者占比分别为0.3%、13.6%、63.0%、23.1%。65.0%患者予以办理入院进一步治疗。救护车送和警察送来院患者的分诊评估情况较其他患者更严重,住院治疗比例更高。结论精神专科医院急诊科不同来院方式患者的分诊评估和诊治情况差异明显,救护车送和警察送来院患者的病情更严重和紧急,在精神科急诊分诊工作中应予以关注。

Objective To investigate the mode of emergency admission and triage evaluation in psychiatric hospital.Methods 1585 patients who were admitted to the Department of Emergency in Grade A psychiatric hospital in Guangzhou fromApril 2017 to November 2017 were selected.A selfmade questionnaire was used to collect the clinical data,admission methods,diagnosis and treatment conditions of the patients.Compared different ways of coming to the hospital.Results Among 1585 patients,the modes of treatment were patients themselves,family members,ambulance and police,accounting for 5.9%,80.9%,3.1%and 10.1%,respectively.The main diagnoses were schizophrenia(368 cases,23.2%),bipolar disorder(357 cases,22.5%),and depression(229 cases,14.4%).The grade of severity of the disease was 0.3%,13.6%,63.0%and 23.1%of the patients with acute,severe,acute and sub-acute cases,respectively.65.0%of the patients were admitted for further treatment.Patients brought to hospital by ambulance and police were assessed more severely than other patients,with a higher rate of hospitalization.Conclusion The triage evaluation and diagnosis and treatment of patients with different ways of coming to the Department of Emergency Psychiatri of psychiatric hospitals are obviously different.Patients sent by ambulance and police are more serious and urgent,which should be paid more attention in the triage work of Department of Emergency Psychiatric.
