间断环形缝合术与8字缝合术在前置胎盘剖宫产子宫下段出血产妇中的应用比较

Comparison of intermittent circular suture and 8-figure suture in the application of placenta previa in caesarean section with hemorrhage in the lower uterine segment

目的比较间断环形缝合术与8字缝合术在前置胎盘剖宫产子宫下段出血产妇中的应用效果。方法选取2018年2月至2019年8月本院收治的前置胎盘剖宫产子宫下段出血产妇106例,依据手术方式不同分为间断环形组(n=53)和8字组(n=53)。8字组予以8字缝合术治疗,间断环形组予以间断环形缝合术治疗,比较两组成功止血率、输血率、子宫切除率及手术情况(止血时间、术中出血量、手术时间、输血量、住院时间)及术后并发症发生率。结果间断环形组成功止血率高于8字组,输血率低于8字组(P<0.05);间断环形组止血时间、手术时间、住院时间短于8字组,术中出血量、输血量低于8字组(P<0.05);间断环形组术后并发症发生率低于8字组(P<0.05)。结论与8字缝合术相比,间断环形缝合术治疗前置胎盘剖宫产子宫下段出血产妇,能提高产妇成功止血率,降低术中出血量、输血量、输血率、术后并发症发生率,缩短止血时间,促进康复。

Objective To compare the effect of interrupted circular suture and 8-figure suture in the treatment of lower uterine bleeding in placenta previa cesarean section.Methods From February 2018 to August 2019,106 cases of postpartum women with placenta previa and caesarean section hemorrhage in our hospital were selected and they were divided into intermittent circular suture group(n=53)and 8-figure group(n=53)according to different surgical methods.The 8-figure group was treated with 8-figure suture,and the intermittent circular suture group was treated with intermittent intermittent annular suture suture.The successful hemostasis rate,blood transfusion rate,hysterectomy rate,operation conditions(hemostasis time,intraoperative blood loss,operation time,blood transfusion amount,hospital stay)and postoperative complication rate between the two groups were compared.Results The successful hemostasis rate of the intermittent circular suture group was higher than the 8-figure group,and the transfusion rate was lower than the 8-figure group(P<0.05).The duration of hemostasis,operation time and hospitalization in the intermittent circular annular suture was shorter than those in the 8-figure group,and the amount of intraoperative blood loss and transfusion were lower than those in the 8-figure group(P<0.05).The incidence of postoperative complications in the intermittent annular suture group was lower than that in the 8-figure group(P<0.05).Conclusion Compared with 8-figure suture,intermittent annular circular suture for the treatment of placenta previa in caesarean section can improve the rate of successful hemostasis,reduce intraoperative blood loss,transfusion volume,transfusion rate and postoperative complication rate,shorten the hemostasis time and promote recovery.
