腹腔镜经脐单孔与三孔治疗结石性胆囊炎对患者胃肠功能及并发症的影响

Effect of laparoscopy on gastrointestinal function and complications in patients with calculous cholecystitis treated by single and triple foramen via umbilicus

目的探讨腹腔镜经脐单孔与三孔治疗结石性胆囊炎对胃肠功能及并发症的影响。方法选取2018年12月至2019年12月本院收治的60例结石性胆囊炎患者作为研究对象,采用随机数表法分为两组,各30例。单孔组采用经脐单孔腹腔镜手术,三孔组采用三孔腹腔镜手术,比较两组临床指标、肠胃功能及并发症发生率。结果单孔组手术时间长于三孔组,视觉模拟评分法(VAS)评分低于三孔组,差异有统计学意义(P<0.05);单孔组排便时间、肠鸣音恢复时间、进食时间、肛门排气时间均短于三孔组,差异有统计学意义(P<0.05);两组术后并发症发生率比较差异无统计学意义。结论与三孔腹腔镜手术相比,经脐单孔腹腔镜手术治疗可促进结石性胆囊炎患者肠胃功能恢复,减轻术后疼痛,且安全性较高,值得临床推广运用。

Objective To investigate the effects of laparoscopy on the gastrointestinal function and complications in the treatment of calculous cholecystitis by single and triple foramen via umbilicus.Methods 60 patients with calculous cholecystitis admitted to our hospital from December 2018 to December 2019 were selected as the research subjects,and they were divided into two groups by random number table method,with 30 cases in each group.The single-port laparoscopic surgery was performed in the single-port group,and the three-port laparoscopic surgery was performed in the three-port group.The clinical indicators,gastrointestinal function and the incidence of complications were compared between the two groups.Results The operation time of the single-port group was longer than that of the three-port group,and the visual analogue scale(VAS)score was lower than that of the three-port group,the difference was statistically significant(P<0.05).The defecation time,bowel sound recovery time,feeding time and anal exhaust time in the single-port group were shorter than those in the three-port group,with statistical significance(P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups.Conclusion Compared with three-port laparoscopic surgery,transumbilical single-port laparoscopic surgery can promote the recovery of gastrointestinal function and relieve postoperative pain in patients with calculous cholecystitis,and it is safe and worthy of clinical application.
