蛇莓和马鞭草在跟骨骨折术后感染中的应用研究

Study on the application of Duchesnea Indica and Verbenae in postoperative infection of calcaneus fracture

目的探讨蛇莓和马鞭草在跟骨骨折术后感染中的应用价值。方法选取2018年6月至2020年9月在本院住院治疗的跟骨骨折术后感染患者60例,根据不同的治疗方式分为实验组和对照组,各30例。实验组患者伤口分泌物标本进行培养、分离和传代,对常见的感染菌株进行流行病学分析。将蛇莓和马鞭草的提取物用不同的比例进行分装并进行浓度递减的体外药敏试验,并确定具体的最小抑菌浓度(minimum inhibitory concentration,MIC),获得体外抗菌谱。对体外药敏试验敏感的患者进行随访记录,对照组患者进行传统的抗感染治疗,比较两组患者的临床效果。结果蛇莓提取液对于革兰氏阴性菌抗菌能力较好,马鞭草提取物对于革兰氏阳性菌抗菌能力较好,二者具有协同互补作用。实验组患者愈合时间、创面缩小率均优于对照组,差异有统计学意义(P<0.05)。结论应用蛇莓和马鞭草提取物治疗跟骨骨折术后感染,可有效抑制感染处的病原体,加快伤口愈合,提高患者的远期生存质量。

Objective To investigate the application value of Duchesnea Indica and Verbenae in postoperative infection of calcaneal fracture.Methods 60 patients with calcaneal fracture after surgery who were hospitalized in our hospital from June 2018 to September 2020 were selected andthey were divided into experimental group and control group according to different treatment methods,with 30 cases in each group.The extracts of Duchesnea Indica and Verbenae were packed in different proportions and the drug sensitivity test with decreasing concentration was carried out in vitro of experimental group,and the specific minimum inhibitory concentration(MIC)was determined to obtain the antimicrobial spectrum in vitro.Follow-up records were made for patients who were sensitive to in vitro drug sensitivity test.In addition,the control group received traditional antiinfection treatment,and the effects between the two groups of patients were compared.Results The extracts of Duchesnea Indica had better antibacterial activity against Gram-negative bacteria,and the extracts of Verbenae had better antibacterial activity against Gram-positive bacteria.The two had synergistic and complementary effects.The healing time and wound reduction rate in the experimental group were better than those in the control group,and the difference was statistically significant(P<0.05).Conclusion Application of Duchesnea Indica and Verbenae extracts treatment of calcaneal fractures of postoperative infection,which can effectively inhibit pathogen infection,accelerate wound healing,improve the quality of the patient's long-term survival.
