KIM1、丛生蛋白联合热休克蛋白72对2型糖尿病肾病诊断的临床意义 被引量:2

Clinical Significance of KIM1,Cluster Protein Combined with Heat Shock Protein 72 in The Diagnosis of Type 2 Diabetic Kidney Disease

摘要 目的研究尿液肾损伤分子1(Kidney Injury Molecule 1,KIM1)、丛生蛋白、热休克蛋白72在2型糖尿病肾病(diabetic kidney disease,DKD)诊断中的应用价值。方法选取我院2018年1月至2019年12月收治的164例2型糖尿病患者,按照有无肾病分为2型糖尿病无肾病组(96例,NDKD组)与2型糖尿病合并肾病组(68例,DKD组),以同期在我院进行健康体检者69例作为对照组(NC组),检测3组的尿液KIM1、丛生蛋白、热休克蛋白72水平,并进一步应用ROS曲线分析尿液KIM1、丛生蛋白、热休克蛋白72对糖尿病肾病的诊断价值。结果尿液KIM1、丛生蛋白、热休克蛋白72水平由高到低为:糖尿病肾病组>糖尿病无肾病组>正常对照组,差异均有统计学意义(P<0.05);ROC曲线分析显示:与各指标单一检测比较,尿液KIM1、丛生蛋白、热休克蛋白72联合检测ROC曲线面积最大(AUC=0.922,P<0.05)。结论尿KIM1、丛生蛋白与热休克蛋白72水平3项指标联合检测对诊断2型糖尿病肾病价值更高,对于该疾病早期治疗具有重要临床意义。 Objective To study the application value of urine kidney injury molecule 1(KIM1),cluster protein and heat shock protein 72 in the diagnosis of type 2 diabetic kidney disease.Methods A total of 164 patients with type 2 diabetes admitted to our hospital from January 2018 to December 2019 were divided into two groups:type 2 diabetes without kidney disease(96 cases,NDKD group)and type 2 diabetes with kidney disease(68 cases,DKD group).Sixty-nine healthy people in our hospital in the same period were taken as control group(NC group),and the urine KIM1,clumping protein and heat shock protein 72 levels of all researchers were detected and the diagnostic value of urine KIM1,cluster protein and heat shock protein 72 in diabetic kidney disease was further analyzed by ROS curve.Results The levels of KIM1,cluster protein and heat shock protein 72 in urine from high to low:diabetic kidney disease group>diabetic without kidney disease group>normal control group,with the differences were statistical significance(P<0.05).The analysis of ROC curve showed that compared with single detection,the combined detection of urine KIM1,cluster protein and heat shock protein 72 had the largest area of ROC curve(area under ROC curve=0.922,P<0.05),which was of higher diagnostic value for diabetic kidney disease.Conclusion Urine KIM1,cluster protein combined with heat shock protein 72 can accurately diagnose type 2 diabetic kidney disease,which has important clinical significance for early treatment of this disease.
