基于老年人潜在不适当用药评价工具基础上的社区高龄慢性病综合管理1例分析

A case study of the comprehensive management of elderly chronic diseases in community based on the evaluation tool of potential inappropriate use of drugs for the elderly

我国已进入老龄化社会,老年人常患多种疾病,多重用药不可避免,通常将患者同时使用5种或以上药物视为多重用药[HOVSTADIUS B,PETERSSON G.Factors leading to excessive polypharmacy[J].Clin Geriatr Med,2012,28(2):159-172.]。本文详细介绍1例从本区三级医院诊治好转出院,后不正规用药致病情加重,通过全科医生团队综合管理,分析患者的多重用药情况,优化治疗方案,历时1年余随访,患者病情稳定,以期对社区全科医生慢性病综合管理、多重用药合理治疗提供参考。

China has entered the population ageing,the elderly often suffer from a variety of diseases,multi-drug inevitable.The use of five or more drugs by a patient at the same time is generally considered to be a multiple use.So how do the basic-level general practitioners do a good job as health gatekeepers for this part of the elderly This article introduces in detail one case who was discharged from the third-level hospitals in this district after receiving treatment and was discharged from the hospital,and then became seriously ill due to improper drug use.Through the Comprehensive Management of the General Practitioner Team,the patients were followed up for more than 1 year,and their condition was stable.The results can be used as a reference for general practitioners'comprehensive management of chronic diseases and rational treatment of multiple drugs.
