不同药物治疗白内障超声乳化术后干眼症的临床效果

Clinical Effect of Different Drugs on Dry Eye after Phacoemulsification of Cataract

目的分析不同药物治疗白内障超声乳化术(PHA)后干眼症的疗效。方法该研究对象为方便选取2018年3月—2020年3月间来院治疗的65例PHA术后干眼症患者。以抽签法为基准,A组33例行玻璃酸钠(SH)滴眼液治疗,B组32例行聚乙烯醇(PVA)滴眼液治疗。对比两组患者临床效果。结果A组的总有效率为96.97%,B组为81.25%,差异有统计学意义(χ^(2)=4.178,P=0.041)。术后1周,A组的干眼症状评分为(1.85±0.16)分,B组为(2.04±0.19)分,差异有统计学意义(t=4.366,P<0.05);A组的严重度评分为(1.54±0.20)分,B组为(1.95±0.24)分,差异有统计学意义(t=7.492,P<0.05)。术后3个月,A组的干眼症状为(0.71±0.04)分,B组为(1.28±0.06)分,差异有统计学意义(t=44.920,P<0.05);A组的严重度评分为(0.49±0.11)分,B组为(0.98±0.17)分,差异有统计学意义(t=13.750,P<0.05)。术后1周与3个月,A组的泪膜破裂时间(BUT)均长于B组(P<0.05);A组的角膜荧光素染色(FLS)评分均低于B组,差异有统计学意义(P<0.05)。术后3个月,A组的泪液分泌试验(SchiemerⅠ)阳性概率为6.06%,B组为25.00%,差异有统计学意义(χ^(2)=4.477,P=0.034)。结论为PHA术后干眼症患者行SH滴眼液治疗可改善其泪膜稳定性和角膜上皮细胞完整性,且能减少泪液分泌量,缓解干眼症状,疗效佳。

Objective To analyze the curative effect of different drugs on dry eye after phacoemulsification(PHA).Methods The object of this study was to conveniently selected 65 patients with dry eye after PHA who came to the hospital for treatment from March 2018 to March 2020.Based on the lottery method,33 patients in group A were treated with sodium hyaluronate(SH)eye drops,and 32 in group B were treated with polyvinyl alcohol(PVA)eye drops.Compared clinical effects of the two groups of patients.Results The total effective rate of group A was 96.97%,and that of group B was 81.25%,and the difference was statistically significant(χ^(2)=4.178,P=0.041).One week after the operation,the dry eye symptom score of group A was(1.85±0.16)points,group B was(2.04±0.19)points,and the difference was statistically significant(t=4.366,P<0.05);the severity score of group A was(1.54±0.20)points,group B was(1.95±0.24)points,and the difference was statistically significant(t=7.492,P<0.05).Three months after the operation,the dry eye symptoms of group A were(0.71±0.04)points,group B was(1.28±0.06)points,and the difference was statistically significant(t=44.920,P<0.05);the severity score of group A was(0.49±0.11)points,group B was(0.98±0.17)points,and the difference was statistically significant(t=13.750,P<0.05).At 1 week and 3 months after operation,the tear film rupture time(BUT)in group A was longer than that in group B(P<0.05);the score of corneal fluorescein staining(FLS)in group A was lower than that in group B,and the difference was statistically significant(P<0.05).Three months after the operation,the positive rate of tear secretion test(SchiemerⅠ)in group A was 6.06%,and that in group B was 25.00%,and the difference was statistically significant(χ^(2)=4.477,P=0.034).Conclusion SH eye drops treatment for patients with dry eye after PHA can improve the stability of the tear film and the integrity of corneal epithelial cells,and can reduce the amount of tear secretion,relieve the symptoms of dry eye,and have a good effect.
