M2a型急性髓细胞性白血病出现血型变异1例

摘要 血型是一种遗传性状,正常情况下不容易改变,受控于第9号染色体的长臂三区四带。但白血病及肿瘤患者由于ABO血型物质的减弱及相关转移酶的抑制或缺失,会发生病理性的血型变异。笔者在浙江大学附属第一医院进修期间遇到M2a型急性髓细胞性白血病(AML-M2a)出现血型变异1例,报道如下:1病历摘要患者,女,18岁,因“停经13周,头晕10天,产检血常规异常1天”于2017年3月14日就诊。WBC 25.0×10^(9)/L,中性粒细胞绝对数21.3×10^(9)/L,Hb 82g/L,PLT 10×10^(9)/L,拟“血三系异常”收住院。患者神清,精神可,贫血貌,全身可见散在瘀点,左眼巩膜有瘀斑,皮肤巩膜无黄染,口腔无渗血,淋巴结未触及肿大,骨髓细胞学显示:原始粒细胞占55%。
