尿酸与脑小血管病负荷的关系

Correlation between uric acid and burden of cerebral small vessel disease

目的分析尿酸和脑小血管病负荷的关系。方法 122例因脑血管病、头晕行头颅磁共振检查及尿酸、肌酐、尿素氮等检查,考虑有脑小血管病患者,依据脑小血管病负荷不同分为0级组(18例)、1级组(34例)、2级组(37例)、3级组(22例)、4级组(11例)。比较不同脑小血管病负荷组患者的基线资料,分析尿酸水平预测脑小血管病的价值。结果随着脑小血管病变负荷增加,尿酸水平越高,不同脑小血管病负荷组患者的尿酸水平比较差异具有统计学意义(P<0.05)。使用ROC曲线预测脑小血管病变发生的价值,结果显示尿酸水平预测脑小血管病ROC曲线下面积为0.6 [95%CI=(0.562, 0.734)],考虑有预测价值,约登指数最大为0.309,尿酸临界值为313μmol/L,尿酸水平对脑小血管病的敏感度和特异度分别为76.7%、62.0%。但本研究病例数偏小,差异无统计学意义(P=0.054>0.05)。结论尿酸与脑小血管病的负荷有关,尿酸水平可能预测脑小血管病的发生。

Objective To analyze the correlation between uric acid and burden of cerebral small vessel disease.Methods There were 122 patients who underwent head magnetic resonance imaging and examinations of uric acid,creatinine,and urea nitrogen due to cerebrovascular disease and dizziness.They were considered to have cerebral small vessel disease,and were divided into level 0 group(18 cases),level 1 group(34 cases),level 2 group(37 cases),level 3 group(22 cases),and level 4 group(11 cases)according to the burden of cerebral small vessel disease.The baseline data of patients with different cerebral small vessel burden was compared,and the value of uric acid level in predicting cerebral small vessel disease was analyzed.Results With the increase of the burden of cerebral small vessel disease,the level of uric acid increased.There was statistically significant difference in uric acid level among patients with different cerebral small vessel burden(P<0.05).The ROC curve was used to predict the occurrence of cerebral small vessel disease.The results showed that the uric acid level predicted the area under the ROC curve of cerebral small vessel disease to be 0.6[95%CI=(0.526,0.734)],which proved its predictive value,with maximum Youden index as 0.309,critical value of uric acid as 417μmol/L,and the sensitivity and specificity of uric acid level to cerebral small vessel disease as 76.7%and 62.0%,respectively.However,the number of cases in this study was small,and the difference was not statistically significant(P=0.054>0.05).Conclusion There is a correlation between uric acid and the burden of cerebral small vessel disease,and the level of uric acid may predict the occurrence of cerebral small vessel disease.
