还原型谷胱甘肽对一氧化碳中毒患者运动功能的影响

摘要 目的观察还原型谷胱甘肽(GSH)对一氧化碳中毒患者运动功能的影响。方法 185例一氧化碳中毒患者,随机分为对照组(103例)和观察组(82例)。对照组采用高压氧、脱水降颅压等常规治疗,观察组在对照组基础上加用GSH治疗。治疗10 d,每天检测四肢肌力、指鼻试验、跟膝胫试验和轮替动作并评分。结果治疗后,观察组下肢肌力、跟膝胫试验恢复率高于对照组,差异具有统计学意义(P<0.05);两组上肢肌力、指鼻试验、轮替动作恢复率比较差异无统计学意义(P>0.05)。观察组上肢肌力、下肢肌力、指鼻试验、轮替动作、跟膝胫试验恢复正常时间分别为(2.91±1.36)、(3.16±1.00)、(2.64±1.03)、(3.48±1.23)、(3.14±1.08)d,均短于对照组的(3.44±1.32)、(3.51±1.07)、(3.60±1.21)、(3.94±1.34)、(3.51±1.11)d,差异具有统计学意义(P<0.05)。观察组上肢肌力、下肢肌力、指鼻试验、轮替动作、跟膝胫试验未恢复患者的改善值均高于对照组,差异具有统计学意义(P<0.05)。结论在高压氧等治疗的基础上适当加用GSH能改善一氧化碳中毒患者肌力下降、共济失调症状并缩短其恢复时间。 Objective To observe the effect of reduced glutathione on motor function in patients with carbon monxide poisoning.Methods A total of 185 patients with carbon monxide poisoning were randomly divided into control group(103 cases)and observation group(82 cases).The control group received conventional therapy of hyperbaric oxygen,dehydration to lower intracranial pressure,and the observation group received GSH therapy on the basis of the control group.After 10 d of treatment,the muscle strength of limbs,finger nose test,heel-knee-shin test and rotation movement were measured and scored every day.Results After treatment,the lower limb muscle strength,heel-knee-shin test recovery rate of the observation group were higher than those of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in upper limb muscle strength,finger-nose test,and rotation recovery rate between the two groups(P>0.05).The recovery time of upper limb muscle strength,lower limb muscle strength,finger nose test,rotation movement and heel-knee-shin test of the observation group were(2.91±1.36),(3.16±1.00),(2.64±1.03),(3.48±1.23)and(3.14±1.08)d,which were shorter than(3.44±1.32),(3.51±1.07),(3.60±1.21),(3.94±1.34)and(3.51±1.11)d of the control group,and the difference was statistically significant(P<0.05).The improvement values of upper limb muscle strength,lower limb muscle strength,finger nose test,rotation movement and heel knee tibia test in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).Conclusion Appropriate addition of reduced glutathione on the basis of hyperbaric oxygen therapy can improve muscle strength,ataxia of patients with carbon monoxide poisoning and shorten their recovery time.
