无症状高尿酸血症对左心功能的影响:一项倾向性评分研究

Effects of Asymptomatic Hyperuricemia on Left Ventricular Function:A Propensity Score Matching Analysis

目的探讨高血压及高血压前期人群中,无症状高尿酸血症对左心功能的影响。方法纳入2019年11月至2020年11月于我院门诊及住院部就诊的高血压及高血压前期患者166例,其中伴有无症状高尿酸血症患者81例,包括男性37例,平均年龄(64.21±8.902)岁,同时选取85例同期就诊于我院的非高尿酸血症者作为对照组,包括男性41例,平均年龄(59.65±9.507)岁。对所有入选患者行超声心动图检查,分析各项指标间是否有统计学差异,以明确无症状高尿酸血症与左心功能之间的关系。结果两组匹配成功各50例。在高血压及高血压前期人群中,无症状高尿酸血症组左房内径增大,左室射血分数、E/A均减低。结论在高血压及高血压前期人群中,无症状高尿酸血症与左室收缩、舒张功能相关,随着血尿酸浓度的升高,左室收缩、舒张功能逐渐减退,为该部分人群的危险因素。

Objective To investigate the effect of asymptomatic hyperuricemia on left ventricular function in patients with hypertension and prehypertension.Methods There were 81 patients with asymptomatic hyperuricemia,including 37 males with an average age of(64.21±8.902)years.At the same time,85 patients with non-hyperuricemia in the same period were selected as the control group,including 41 males with an average age of(59.65±9.507)years.According to the relevant diagnostic criteria of the 2019 edition of Chinese guidelines for the Prevention and treatment of Hypertension and the Chinese expert consensus on the recommendations for the diagnosis and treatment of asymptomatic hyperuricemia complicated with Cardiovascular Diseases,patients with prehypertension,grade 1 and 2 hypertension and asymptomatic hyperuricemia were included.All the selected patients were examined by echocardiography to analyze whether there were statistical differences among various indexes in order to clarify the relationship between asymptomatic hyperuricemia and left ventricular function.Results There were 50 cases of successful matching in each of the two groups.In patients with hypertension and prehypertension,left atrial diameter increased,left ventricular ejection fraction(LVEF)and E/A decreased in asymptomatic hyperuricemia group.Conclusion In patients with hypertension and prehypertension,asymptomatic hyperuricemia is associated with left ventricular systolic and diastolic function,and the left ventricular systolic and diastolic function decreases gradually with the increase of serum uric acid concentration,which is a risk factor for this part of the population.
