血清半乳糖凝集素-3在高龄老人慢性心力衰竭诊疗中的临床价值

Clinical Value of Serum Galectin-3 in the Diagnosis and Treatment of Chronic Heart Failure in Elderly Patients

目的研究80岁以上高龄老人血清半乳糖凝集素-3(galectin-3,Gal-3)水平在慢性心力衰竭诊疗中的临床价值。方法选取研究对象为内蒙古自治区人民医院2019年12月至2020年10月接诊的符合慢性心力衰竭诊断标准的高龄患者(80岁以上)90例设为实验组,选取同期接诊的无心力衰竭高龄患者90例设为对照组。结果(1)实验组与对照组患者HDL、H-FABP、Hs-CRP、IMT、颈动脉斑块数量、右心室舒张径、LAD、LVEF、NT-pro BNP、Gal-3等临床指标具有显著差异,其余指标无统计学意义;(2)实验组内比较不同心功能分级(NYHA)之间年龄、斑块数量、右心室舒张径、左心室收缩末期径、LVEF、NT-pro BNP及Gal-3差异具有统计学意义;(3)Gal-3、NT-pro BNP、LVESD、右心室舒张径与慢性心力衰竭的NYHA心功能分级之间具有正向相关关系,LVEF与其具有反向相关关系,斑块数量及年龄与其无明显相关关系。结论血清Gal-3浓度与高龄老人慢性心力衰竭的发生及其严重程度具有一定相关性,对高龄慢性心衰患者的诊疗具有重要价值。

Objective To investigate the clinical value of serum galectin-3(Gal-3)level in the diagnosis and treatment of chronic heart failure(CHF)in elderly people over 80 years old.Methods 90 elderly patients(over 80 years old)who met the diagnostic criteria of chronic heart failure admitted to Inner Mongolia Autonomous Region People’s Hospital from December 2019 to October 2020 were selected as the experimental group,and 90 elderly patients without heart failure admitted to Inner Mongolia Autonomous Region during the same period were selected as the control group.Results(1)There were significant differences in HDL,H-FABP,hs-CRP,IMT,carotid plaque number,right ventricular diastolic diameter,LAD,LVEF,NT-Pro BNP,Gal-3 and other clinical indicators between the experimental group and the control group,while the other indicators were not statistically significant.(2)The differences in age,plaque number,right ventricular diastolic diameter,left ventricular end-systolic diameter,LVEF,NT-Pro BNP and Gal-3 among different cardiac function grades(NYHA)in the experimental group were statistically significant.(3)Gal-3,NT-pro BNP,LVESD,and right ventricular diastolic diameter were positively correlated with NYHA cardiac function grade of chronic heart failure,while LVEF was negatively correlated with it,and plaque number and age were not significantly correlated with it.Conclusion Serum Gal-3 concentration has a certain correlation with the occurrence and severity of chronic heart failure in elderly patients,which is of great value for the diagnosis and treatment of elderly patients with chronic heart failure.
