腰大池持续引流治疗交通性脑积水对患者脑脊液廓清速率、脑积水及并发症发生率的影响

Effect of continuous drainage of lumbar cistern in treatment of communicative hydrocephalus on the rate of cerebrospinal fluid clearance,hydrocephalus and complication

目的于交通性脑积水患者中实施腰大池持续引流治疗对并发症发生率、脑积水、脑脊液廓清速率的影响进行探究。方法对2019年1月~2020年2月的60例急性交通性脑积水患者做分组(随机数表法)研究,分为甲组与乙组,两组均30例患者。乙组连续多次腰穿引流治疗,甲组腰大池持续引流治疗,比较并发症发生率、脑积水情况、脑脊液廓清速率。结果不同治疗干预实施后,甲组并发症发生率低于乙组(P<0.05);甲组症状脑积水率低于乙组(P<0.05);甲乙两组的静止脑积水率间差异不显著(P>0.05);甲组急性脑积水缓解率高于乙组(P<0.05);甲组脑脊液廓清速率高于乙组(P<0.05)。结论于交通性脑积水患者中,实施腰大池持续引流治疗的安全性高,可有效缓解交通性脑积水、血性脑脊液快速廓清,并降低继发性交通性脑积水的风险。

Objective To investigate the effect of continuous drainage of lumbar cistern in patients with communicative hydrocephalus on the incidence of complications,hydrocephalus and cerebrospinal fluid clearance rate.Methods A grouping study(random number table method)of 60 patients with acute communicating hydrocephalus from January 2019 to February 2020 was divided into group A and group B,with 30 patients in both groups.Group B was treated with continuous lumbar puncture and drainage,and group A was treated with continuous drainage of lumbar cistern.The incidence of complications,hydrocephalus,and cerebrospinal fluid clearance rate were compared.Results The incidence of complications in group A was lower than that in group B(P<0.05).The rate of symptomatic hydrocephalus in group A was lower than that in group B(P<0.05).There was no significant difference in the rate of static hydrocephalus between the two groups(P>0.05).The remission rate of acute hydrocephalus in group A was higher than that in group B(P<0.05).The rate of CSF clearance in group A was higher than that in group B(P<0.05).Conclusion In patients with communicating hydrocephalus,continuous drainage of the lumbar cistern is safer and can effectively alleviate communicating hydrocephalus and rapid clearance of blood cerebrospinal fluid,and reduce the risk of secondary communicating hydrocephalus.
