血清维生素D、CXCL13水平与COPD患者肺功能的相关性分析

摘要 目的探究慢性阻塞性肺疾病(COPD)患者血清维生素D、趋化因子配体13(CXCL13)水平与其肺功能的相关性。方法将2018年1月~2020年1月接受治疗的60例COPD患者设为研究组,另选取同期接受体格检查的50例受试者为对照组,对比两组受试者血清维生素D、CXCL13水平,以及第1秒用力呼气容积占预计值百分比(FEV1/预计值)、第1秒用力呼气容积占比用力肺活量的百分比(FEV1/FVC)差异,并分别计算血清维生素D、CXCL13同FEV1/预计值、FEV1/FVC相关性。结果研究组患者CXCL13水平明显高于对照组,血清维生素D、FEV1/预计值以及FEV1/FVC均明显低于对照组(P<0.05);COPD患者血清维生素D、FEV1/预计值以及FEV1/FVC明显高于AECOPD患者,CXCL13明显低于AECOPD患者(P<0.05);Spearman相关性分析显示,血清维生素D同FEV1/预计值、FEV1/FVC均呈正相关(P<0.05),CXCL13同FEV1/预计值、FEV1/FVC均呈负相关(P<0.05)。结论COPD患者血清维生素D水平明显降低、CXCL13水平升高,其水平同COPD患者肺功能明显相关,可考虑将血清维生素D、CXCL13作为COPD患者肺功能评估指标应用于临床。 Objective To explore the correlation between serum vitamin D,chemokine ligand 13(CXCL13)and lung function in patients with chronic obstructive pulmonary disease(COPD).Methods 60 patients with COPD who were treated in our hospital from January 2018 to January 2020 were selected as the study group,and 50 individuals who received physical examination in our hospital at the same time were selected as the control group.The serum vitamin D and CXCL13 levels of the two groups were compared,as well as the difference between the first-second forced expiratory volume(FEV1/predicted value)and the first-second forced expiratory volume(FEV1/FVC).Results Compared with the control group,CXCL13 in the study group was significantly higher,while serum vitamin D,FEV1/predicted value and FEV1/FVC were significantly lower(P<0.05).The comparison showed that serum vitamin d,FEV1/predicted value and FEV1/FVC of COPD patients were significantly higher than those of AECOPD patients,while CXCL13 was significantly lower than that of AECOPD patients(P<0.05).Spearman correlation analysis showed that serum vitamin D was positively correlated with FEV1/predicted value and FEV1/FVC(P<0.05),while CXCL13 was negatively correlated with FEV1/predicted value and FEV1/FVC(P<0.05).Conclusion The serum vitamin d level will obviously decrease and CXCL13 level will increase in COPD patients.The above factors are obviously related to the pulmonary function of COPD patients.It is considered that serum vitamin d and CXCL13 can be used as indicators for evaluating pulmonary function of COPD patients in clinic.
