腹部温通针刺法对顽固性面神经麻痹患者肌电图与面神经功能的影响

Effect of Acupuncture with Warming and Ventilating Abdomen on Electromyography and Facial Nerve Function in Patients with Intractable Facial Nerve Palsy

目的观察临床应用"温通针法"治疗气虚血瘀型顽固性面神经麻痹治疗的疗效,研究"温通针法"的临床优势。方法选取2018年10月—2019年10月期间就诊于沈阳市中医院针灸科住院及门诊的周围性面神经麻痹患者60例,按就诊顺序随机分为对照组和治疗组,每组各30例。治疗组患者使用"温通针法"进行干预,对照组患者则使用常规针刺干预,两组均治疗2个月。评估肌电图、H-B面神经分级量表、FDI调查问卷评价在治疗前后的变化,评价疗效差异。结果经"温通针法"治疗后,治疗组肌电图RI潜伏期明显减小(P<0.05)、Cmap明显提高(P<0.05);H-B面神经功能分级变化明显且总有效率显著提高(P<0.05);FDI躯体功能评分显著提高(P<0.05)、FDI社会功能评分显著降低(P<0.05),且上述指标改善情况均优于对照组。结论 "温通针法"治疗气虚血瘀型顽固性面神经麻痹疗效显著,在肌电图RI、Cmap、H-B面神经功能分级、FDI躯体功能评分、FDI社会功能评分的改善作用优于常规针刺法。

Objective To observe the clinical application of "Wen Tong Acupuncture" in the treatment of intractable facial nerve palsy of Qi deficiency and blood stasis,and to study the clinical advantages of "Wen Tong Acupuncture".Methods From October 2018 to October 2019,60 patients with peripheral facial paralysis who were admitted to the Acupuncture Department of Shenyang Hospital of Traditional Chinese Medicine and Moxibustion were selected.They were randomly divided into control group and treatment group according to the order of treatment,with 30 cases in each group.Patients in the treatment group used "Wen Tong Acupuncture Method" for intervention,and patients in the control group used conventional acupuncture intervention.Both groups were treated for 2 months.Evaluate the changes of electromyography,H-B facial nerve grading scale,FDI questionnaire before and after treatment,and evaluate the difference in curative effect.Results After the treatment with "Wen Tong Acupuncture",the EMG RI latency of the treatment group was significantly reduced(P <0.05),Cmap was significantly improved(P <0.05);HB facial nerve function classification changed significantly and the total effective rate was significantly increased(P <0.05);FDI physical function score was significantly improved(P <0.05),FDI social function score was significantly reduced(P <0.05),and the improvement of the above indicators was better than that of the control group.Conclusion "Wen Tong Acupuncture" is effective in treating refractoiy facial nerve palsy of Qi deficiency and blood stasis type.The improvement effect of EMG RI,Cmap,HB facial nerve function classification,FDI physical function score,FDI social function score is better than conventional acupuncture.
