负压封闭引流治疗骨科创伤感染的效果及对炎症标志物的影响

Effect of Vacuum Sealing Drainage in the Treatment of OrthopedicTrauma Infection and its Influence on Inflammatory Markers

目的探讨负压封闭引流治疗骨科创伤感染的效果及对炎症标志物的影响。方法回顾性选取2018年1月—2020年1月医院骨科创伤感染患者48例,依据治疗方法分为常规换药治疗组(常规组,n=24)和常规换药治疗基础上负压封闭引流(vacuum sealing drainage,VSD)技术治疗组(VSD组,n=24)两组,统计分析两组患者的临床疗效、VAS评分、炎症标志物水平、换药次数、创面愈合时间、住院时间、骨折愈合时间。结果VSD组的治疗有效率为95.8%(23/24),高于常规组的75.0%(18/24),差异有统计学意义(P<0.05)。VSD组患者治疗后较治疗前的VAS评分、CRP、WBC、ESR水平降低幅度均显著高于常规组,差异有统计学意义(P<0.05)。VSD组患者的换药次数显著少于常规组,差异有统计学意义(P<0.05),创面愈合时间、住院时间、骨折愈合时间均显著短于常规组,差异有统计学意义(P<0.05)。结论骨科创伤感染VSD治疗的疗效显著,能够有效降低炎症标志物水平。

Objective To investigate the effect of vacuum sealing drainage in the treatment of orthopedic trauma infection and its influence on inflammatory markers.Methods 48 cases of patients with orthopedic trauma infection in our hospital from January 2018 to January 2020 were retrospectively selected.According to the treatment methods,they were divided into two groups:conventional treatment group(conventional group,n=24)and vacuum sealing drainage(VSD)technology based on the conventional dressing change group(VSD group,n=24).The clinical efficacy,VAS scores,inflammatory factor levels,dressing change times,wound healing time and hospitalization time,fracture healing time of the two groups were statistically analyzed.Results In terms of effective rate,the VSD group was 95.8%(23/24)and the conventional group was 75.0%(18/24),the former was significantly higher than the latter,the difference was statistically significant(P<0.05).After treatment,the decreases of VAS score,CRP,WBC and ESR levels of the VSD group were significantly higher than the conventional group,the difference was statistically significant(P<0.05).The times of dressing change of the VSD group was significantly shorter,the difference was statistically significant(P<0.05),the time of wound healing,hospitalization and fracture healing were significantly shorter than the conventional group,the difference was statistically significant(P<0.05).Conclusion VSD is effective in the treatment of orthopedic trauma infection and can effectively reduce the level of inflammatory factors.
