降脂灵胶囊联合曲美他嗪对冠心病合并心力衰竭患者血管内皮功能和心功能的影响

Effects of Jiangzhiling capsules combined with trimetazidine on vascular endothelial function and heart function in patients with coronary heart disease complicated with heart failure

目的探究降脂灵胶囊联合曲美他嗪对冠心病合并心力衰竭(简称心衰)患者血管内皮功能和心功能的影响。方法用随机数字表法将2018年4月至2019年4月本院心内科收治的125例冠心病合并心衰患者分为2组。在常规综合治疗基础上,对照组服用曲美他嗪,研究组服用曲美他嗪和降脂灵胶囊。比较2组持续治疗8周后主要临床症状评分、临床效果、血管内皮功能和心功能。结果与对照组比,研究组治疗后在胸闷、乏力、喘息、气促方面症状评分明显下降(P<0.01),临床显效率和总有效率明显升高(P<0.01),血清6-酮6-前列腺素FIα水平和肱动脉内皮依赖性血管舒张功能明显增大,内皮素-1水平下降(P<0.01),心功能指标左室射血分数、左心室短轴缩短率明显增大,左心室舒张末期内径明显缩小(P<0.01)。结论降脂灵胶囊与曲美他嗪联合治疗冠心病合并心衰疗效确切,可有效缓解心衰临床症状,改善血管内皮功能和心功能。

Objective To explore the effect of Jiangzhiling capsule combined with trimetazidine on vascular endothelial function and heart function in patients with coronary heart disease complicated with heart failure(abbreviated as heart failure).Methods From April 2018 to April 2019,125 patients with coronary heart disease complicated with heart failure admitted to the cardiology department of our hospital were divided into two groups using a random number table.On the basis of conventional comprehensive treatment,the control group was treated with trimetazidine,and the study group was treated with trimetazidine and Jiangzhiling capsules.The main clinical symptom scores,clinical effects,vascular endothelial function and cardiac function were compared between the two groups after 8 weeks of continuous treatment.Results Compared with the control group,the symptom scores of the study group in terms of chest tightness,fatigue,wheezing,and shortness of breath decreased significantly after treatment(P<0.01),and the clinical efficacy and total effective rate were significantly increased(P<0.01).Serum 6-ketoPGF1αlevel and FMD increased significantly,ET-1 level decreased(P<0.01),cardiac function indexes LVEF and FS increased significantly,and LVEDD decreased significantly(P<0.01).Conclusion The combination of Jiangzhiling capsule and trimetazidine in the treatment of coronary heart disease complicated with heart failure is effective,which can effectively relieve the clinical symptoms of heart failure,improve the function of vascular endothelium and heart function of patients.
