联合检测miR⁃195、SOCS3 mRNA、miR⁃210对急性淋巴细胞性白血病的诊断价值分析 被引量:4

摘要 目的:研究联合检测微小核苷酸195(miR⁃195)、细胞因子信号转导抑制因子⁃3(SOCS3 mRNA)、微小核苷酸210(miR⁃210)在急性淋巴细胞性白血病(ALL)诊断及危险度评估中的价值。方法:选取2014年2月至2017年2月收治的120例ALL患者为研究对象,初诊患者74例、缓解组患者22例、复发患者24例,另选取健康体检的儿童120例作为对照组,分别对不同组别研究对象的miR⁃195、SOCS3 mRNA、miR⁃210表达水平,初诊组患者不同危险度与miR⁃195、SOCS3 mRNA、miR⁃210表达水平的相关性,初诊组患者危险度的危险因素以及单独检测和联合检测对ALL疾病的诊断效能进行分析。结果:对照组、缓解组、复发组、初诊组以及不同危险度患者的miR⁃195、SOCS3 mRNA、miR⁃210表达水平差异明显(P<0.05)。经两两比较,各组miR⁃195、miR⁃210表达水平从高到低依次为对照组、缓解组、初诊组、复发组以及标危组、中危组、高危组,SOCS3 mRNA表达水平从高到低依次为复发组、初诊组、缓解组、对照组以及高危组、中危组和标危组(P<0.05)。患者的危险度与miR⁃195、miR⁃210表达水平呈现负相关,与SOCS3 mRNA呈现正相关,联合检测灵敏度显著高于单独检测患者的miR⁃195、miR⁃210、SOCS3 mRNA的标危、中危以及高危临界值分别为2.43、2.01、1.34;6.32、5.44、4.09;0.66、0.98、1.22。结论:联合检测miR⁃195、SOCS3 mRNA、miR⁃210在ALL患者诊断中灵敏度较高,且miR⁃195、SOCS3 mRNA、miR⁃210表达水平与ALL患者危险度呈显著相关,建议在临床中推广。 Objective:To determine the value of combined detection of microRNA 195(miR⁃195),suppressor of cytokine signaling⁃3(SOCS3)mRNA,and miR⁃210 in diagnosis and risk assessment of acute lymphocytic leukemia(ALL).Methods:A total of 120 ALL patients who stayed in our hospital between February 2014 and February 2017 were included as subjects,comprising 74 in newly diagnosed group,22 in remission group,and 24 in relapse group.In addition,120 children health checkup subjects in our hospital were selectedas controls.All groups were examined for the expression levels of miR⁃195,SOCS3 mRNA,and miR⁃210.The correlation between the risk assessment and the expression of miR⁃195,SOCS3 mRNA and miR⁃210in newly diagnosed group was determined.Risk factors of the risk assessment in newly diagnosed group were analyzed.The diagnostic efficacy of individual and combined testing in ALL was investigated.Results:The expression levels of miR⁃195,SOCS3 mRNA and miR⁃210 differed significantly among the control group,remission group,relapse group,newly diagnosed group and patients with different risk assessment(P<0.05).Pairwise comparison revealed that ranking of groups with expression levels of miR⁃195 and miR⁃210 from high to low were control group,remission group,newly diagnosed group,and relapse group;or in the newly diagnosed patients,standard⁃risk group,medium⁃risk group and high⁃risk group.The ranking of groups with expression level of SOCS3 mRNA from high to lowwere relapse group,newly diagnosed group,remission group,control group;or in the newly diagnosed patients,high⁃risk group,medium⁃risk group and standard⁃risk group(P<0.05).The patients′risk assessment was negatively correlated with the expression levels of miR⁃195 and miR⁃210,and positively correlated with SOCS3 mRNA.The combined detection yielded significantly higher sensitivity than did individual detection of the micro⁃RNA.By ROC curve analysis,the cut⁃off valuesin determining the standard,medium and high risks for of miR⁃195were 2.43,2.01 and 1.34,for miR⁃210 were 6.32,5.44,and 4.09,for SOCS3 mRNA were 0.66,0.98,and 1.22,respectively.Conclusion:Combined detection of miR⁃195,SOCS3 mRNA,and miR⁃210 is highly sensitive in diagnosing ALL.The expression levels of miR⁃195,SOCS3 mRNA,and miR⁃210 are significantly related to the risk assessment in ALL patients,and therefore are recommendable for use in clinical settings.
