针灸并用治疗溃疡性结肠炎Meta分析及试验序贯分析

Meta-Analysis and Trial Sequential Analysis of Acupuncture Combined with Moxibustion in the Treatment of Ulcerative Colitis

目的系统评价针灸并用与柳氮磺吡啶(SASP)治疗溃疡性结肠炎(Ulcerativecolitis,UC)的疗效差异。方法计算机检索中国期刊全文数据库(CNKI)、万方数据库(Wan Fang)、重庆维普中文科技期刊数据库(VIP)、中国生物医学文献数据库网络版(CBM)以及Pub Med、EMbase和The Cochrane library数据库中有关针灸并用治疗UC的所有随机对照试验(RCTs),检索时间为建库至2020年2月24日。由两名研究者独立地进行文献筛选与资料提取,对纳入研究进行质量评价,采用Rev Man 5.3软件进行Meta分析。结果总共纳入12项RCTs,合计970例UC患者。Meta分析结果显示,总有效率方面:OR=7.32,95%CI(4.58,11.70),P<0.00001;治疗后便常规正常患者所占比例:OR=4.31,95%CI(1.96,9.48),P=0.0003。不良反应发生率方面:OR=0.17,95%CI(0.08,0.37),P=0.0001。结论针灸并用治疗UC的疗效优于SASP,且安全性较高,但由于纳入的文献质量较低,仍需更多设计严谨的多中心、大样本RCTs研究加以验证。

Objective To investigate the key target and mechanism of Lonicerae Japonicae in the treatment of Ig A Nephropathy(Ig AN)based on network pharmacology.Methods The effective active components and potential targets of Lonicerae Japonicae were screened out through the Chinese medicine systematic pharmacology database and analysis platform(TCMSP),the disease targets of Ig A nephropathy were collected through Genescard and OMIM databases,and the gene names of target proteins relative to each other were searched using Uniprot database.Using Cytoscape 3.6.0 build honeysuckle drug in the treatment of Ig A nephropathy-composition-targets-disease network,using the String database network construction of honeysuckle and Ig A nephropathy target protein interaction network,and using R software key targets for gene ontology(GO)and KEGG pathway enrichment analysis,further explore the mechanism of action of honeysuckle in the treatment of Ig A nephropathy and curative effect.Results 23 active components and 365 potential target genes were obtained,among which 66 overlaps with target genes of Ig A nephropathy,including IL6,ALB,VEGF,AKT1,and PTGS2.Gene enrichment analysis results suggested that Honeysuckle may be mainly involved in immune regulation,inflammatory response,oxidative stress,apoptosis and other biological processes to play a role in the treatment of Ig A nephropathy.Conclusion Based on network pharmacology research,the active components and potential targets of Lonicerae Japonicae in the treatment of Ig A nephropathy were preliminarily revealed,providing basis and theoretical support for further research on the mechanism of action of honeysuckle in the treatment of Ig A nephropathy.
