摘要
目的:探讨经外科手术病理检查与胃镜下活体组织检查在胃癌病情评估中应用价值。方法:本次研究中所纳入的研究对象为商丘市中医院医治的100胃癌患者,选取的时间段为2018年5月—2020年5月,所有研究对均分别进行胃镜下活体组织检查、外科手术病理检查。以手术病理检查结果为金标准,观察胃镜下活体组织检查的检出率、病态形态及分化程度等相关情况。结果:100例患者中,手术病理检查结果为100.00%,胃镜下活体组织检出率为94.00%,疑似率4.00%、未确诊率2.00%。胃镜下活体组织检查胃癌病变形态BorrmannⅢ型9.00%、Ⅳ型3.00%的检出率,明显低于手术病理检出率的19.00%、10.00%(P<0.05)。胃镜下活体组织分化型检出率、分化不良型检出率分别为62.00%、13.00%,明显低于手术病理检出率76.00%、24.00%(P<0.05)。结论:胃镜下活体组织检查诊断胃癌具有较高的诊断率,但全面评估胃癌患者病情时,仍需以外科手术病理诊断结果为主,才可为临床治疗提供更为准确的参考依据。
Objective: To explore the application value of surgical pathology and gastroscope biopsy in the evaluation of gastric cancer. Methods: 100 gastric cancer patients in the hospital from May 2018 to May 2020 were selected as research objects. All subjects underwent gastroscopic biopsy and surgical pathological examination. The results of surgical pathological examinations were taken as the gold standard, the detection rate, pathological morphology and differentiation degree of living tissue examination under gastroscope were observed. Results: In 100 patients, the results of surgical pathology were 100.00%, the detection rate of living tissue under gastroscope was 94.00%, the suspected rate was 4.00%, and the rate of undetermined diagnosis was 2.00%. The detection rates of Borrmann type Ⅲ and type Ⅳ 3.00% of gastric cancer lesion morphology under gastroscope biopsy were significantly lower than the 19.00% and 10.00% detection rates of surgical pathology(P<0.05). The detection rate of differentiated type and poorly differentiated type under gastroscopy were 62.00% and 13.00% respectively, which were significantly lower than the76.00% and 24.00% detection rates of surgical pathology(P<0.05). Conclusion: Endoscopic biopsy has a high diagnostic rate in the diagnosis of gastric cancer, but the overall evaluation of gastric cancer patients still needs surgical pathological diagnosis results, which can provide more accurate reference for clinical treatment.
