基于快速康复外科理念护理在急性乳腺炎化脓穿刺切开冲洗患者中的应用

Application of nursing based on the concept of fast track surgery in patients with acute mastitis

目的探讨基于快速康复外科理念(FTS)的护理在急性乳腺炎化脓穿刺切开冲洗患者中的应用效果。方法选择2018年6月-2020年6月九江市第一人民医院收治的60例急性乳腺炎化脓患者,按随机数字表法分为2组,对照组(30例)采取常规护理,观察组(30例)实施FTS护理。比较2组康复情况(脓肿消退时间、白细胞计数恢复至正常时间、住院时间),干预前和干预7 d后疼痛程度,调查患者对护理满意度。结果观察组脓肿消退时间(3.53±0.67)d、白细胞计数恢复至正常时间(3.04±0.65)d、住院时间(9.37±1.18)d,均短于对照组的(4.89±1.02)d、(4.52±1.01)d、(12.56±2.34)d,有统计学差异(P<0.05)。观察组干预后疼痛程度轻于对照组,有统计学差异(P<0.05)。观察组护理满意度中操作技术(22.24±1.01)分、服务态度(22.37±1.04)分、健康指导(22.39±1.02)分、心理疏导(22.65±1.12)分,均高于对照组的(20.03±1.21)分、(20.09±1.23)分、(20.05±1.22)分、(20.10±1.67)分,有统计学差异(P<0.05)。结论FTS护理能够促进急性乳腺炎化脓穿刺切开冲洗患者康复,减轻患者疼痛程度,从而提升护理满意度。

Objective To explore the application effect of nursing based on the concept of fast track surgery(FTS)in patients with acute mastitis.Methods A total of 60 patients with acute mastitis and suppuration admitted to the First People’s Hospital of Jiujiang City from June 2018 to June 2020 were selected and divided into two groups according to random number table method.The control group(30 cases)received routine nursing,and the observation group(30 cases)received FTS nursing.The recovery situation(abscess subsidence time,white blood cell count return to normal time,hospitalization time),pain degree before intervention and 7 d after intervention were compared between the two groups,and patients’satisfaction with nursing was investigated.Results The abscess resolution time,white blood cell count returned to normal time and hospitalization time in the observation group were(3.53±0.67)d,(3.04±0.65)d and(9.37±1.18)d,shorter than those in the control group(4.89±1.02)d,(4.52±1.01)d and(12.56±2.34)d,with statistical difference(P<0.05).The pain degree of the observation group was less than that of the control group after intervention,with statistical difference(P<0.05).The scores of operating technique(22.24±1.01)points,service attitude(22.37±1.04)points,health guidance(22.39±1.02)points and psychological counseling(22.65±1.12)points in nursing satisfaction of the observation group were higher than those of the control group(20.03±1.21)points,(20.09±1.23)points,(20.05±1.22)points,(20.10±1.67)points,with statistical difference(P<0.05).Conclusion FTS nursing can promote the recovery of patients with acute mastitis suppurative puncture incision and irrigation,reduce the pain degree of patients,and improve nursing satisfaction.
