超声引导下双侧腹横肌平面阻滞联合右美托咪定、舒芬太尼在开腹胃癌及结肠癌根治术中的镇痛效果

Analgesic effect of ultrasound-guided bilateral transversus abdominis plane block combined with dexmedetomidine and sufentanil in open radical operation for gastric cancer and colon cancer

目的探索超声引导下双侧腹横肌平面阻滞联合右美托咪定、舒芬太尼在开腹胃癌及结肠癌根治术中的镇痛效果。方法将90例行开腹胃癌及结肠癌根治术患者根据单双号随机化分为对照组和观察组,各45例。对照组采用超声引导下双侧腹横肌平面阻滞联合舒芬太尼干预,观察组采用超声引导下双侧腹横肌平面阻滞联合右美托咪定、舒芬太尼干预。比较两组的镇痛效果。结果观察组术后各时间段的HR、MAP稳定情况优于对照组(P<0.05)。观察组术后3、6、12、24 h的VAS评分均低于对照组(P<0.05)。术后24、48 h,观察组的情绪和睡眠质量评分均低于对照组(P<0.05)。结论对开腹胃癌及结肠癌根治术患者实施超声引导下双侧腹横肌平面阻滞联合右美托咪定、舒芬太尼干预的效果显著。

Objective To explore the analgesic effect of ultrasound-guided bilateral transversus abdominis plane block combined with dexmedetomidine and sufentanil in open radical operation for gastric cancer and colon cancer.Methods Ninety patients who underwent open radical operation for gastric cancer and colon cancer were divided into control group and observation group according to single and double randomized,with 45 cases in each group.The control group was treated with ultrasound-guided bilateral transversus abdominis plane block combined with sufentanil,and the observation group was treated with ultrasound-guided bilateral transversus abdominis plane block combined with dexmedetomidine and sufentanil.The analgesic effect of the two groups was compared.Results The stability of HR and MAP at each time period after surgery of the observation group were better than those of the control group(P<0.05).The VAS scores at 3,6,12 and 24 h after operation in the observation group were lower than those in the control group(P<0.05).At 24 and 48 h after operation,the scores of mood and sleep quality in the observation group were lower than those in the control group(P<0.05).Conclusion The ultrasound-guided bilateral transversus abdominis plane block combined with dexmedetomidine and sufentanil in patients with open radical operation for gastric cancer and colon cancer has a significant effect.
