CT与高频超声对不同瘤体直径的腮腺混合瘤患者诊断价值分析

摘要 目的:对比分析CT与高频超声对不同瘤体直径的腮腺混合瘤患者诊断价值。方法:分析本院2017年1月至2020年4月收治的85例腮腺肿瘤患者的病案资料,术前CT和高频超声两种方式检查,比较CT与高频超声诊断腮腺混合瘤总体效能、CT与高频超声诊断瘤体直径>2 cm的腮腺混合瘤效能、CT与高频超声诊断瘤体直径≤2 cm的腮腺混合瘤效能。结果:CT、高频超声诊断腮腺混合肿瘤总体效能(灵敏度、特异度、阳性预测值、阴性预测值、符合率)无显著性差异(P>0.05),高频超声诊断瘤体直径>2 cm的腮腺混合瘤灵敏度、特异度、阳性预测值、阴性预测值、符合率较CT检查均显著提高(P<0.05),CT诊断瘤体直径≤2cm的腮腺混合瘤的灵敏度、特异度、阳性预测值、阴性预测值及符合率较高频超声均显著提高(P<0.05)。结论:高频超声可作为腮腺混合瘤的常规筛查方法,若瘤体直径<2cm,则应该联合CT综合诊断,提高腮腺混合瘤的诊断效能。 Objective:To compare and analyze the diagnostic value of CT and high-frequency ultrasound in patients with mixed parotid gland tumors with different tumor diameters.Method:To analyze the medical record data of 85 patients with parotid gland tumors admitted to our hospital from January 2017 to December 2020,preoperatively CT and high-frequency ultrasound examinations,compare the overall effectiveness of CT and high-frequency ultrasound in diagnosing mixed parotid gland tumors,the effectiveness of CT and high-frequency ultrasound in diagnosing mixed parotid gland tumors with a tumor diameter>2cm,and the diagnosis of tumor diameter by CT and high-frequency ultrasound≤Efficiency of 2cm mixed parotid gland tumor.Results:CT and high-frequency ultrasound diagnosis of mixed parotid gland tumors have no significant difference in overall efficacy(sensitivity,specificity,positive predictive value,negative predictive value,coincidence rate)(P>0.05),high-frequency ultrasound.The sensitivity,specificity,positive predictive value,negative predictive value,and coincidence rate of diagnosing mixed parotid gland tumor with tumor diameter>2cm were significantly higher than those of CT examination(P<0.05),and the sensitivity of CT in diagnosing mixed parotid gland tumor with tumor diameter≤2cm.The specificity,positive predictive value,negative predictive value and coincidence rate of higher frequency ultrasound were significantly improved(P<0.05).Conclusion:High frequency ultrasound can be used as a routine screening method for parotid mixed tumors.If the diameter of the tumor is less than 2cm,it should be combined with CT comprehensive diagnosis to improve the diagnostic efficiency of mixed parotid gland tumors.
