摘要
目的:探讨分析肌力训练与耐力训练治疗卒中后痉挛期患者效果。方法:选取2019年2月~2020年8月收治的102例脑卒中后痉挛期患者分为观察组和对照组,对照组给予常规康复训练,观察组给予肌力训练与耐力训练治疗,比较两组患者治疗前后FMA、Berg、FIM及MAS评分,在角速度为60°/s时两组患者肩、膝屈伸生物力学指标水平,临床疗效。结果:治疗前两组患者FMA、Berg、FIM及MAS评分无差异(P>0.05),治疗后观察组患者FMA、Berg、FIM评分上升更显著(P<0.05),治疗后两组患者MAS评分对比无显著性差异(P>0.05)。在角速度为60°/s时治疗前两组患者肩、膝屈伸生物力学指标水平无显著差异(P>0.05),治疗后两组患者肩、膝屈伸生物力学指标水平均显著提高,且观察组患者指标改善更显著(P<0.05)。观察组患者临床疗效总有效率为96.08%,较对照组82.35%显著提高(P<0.05)。结论:肌力训练联合耐力训练可提高脑卒中后痉挛期患者肢体运动功能,维持运动平衡功能,促进日常生活能力的恢复,值得推广。
Objective:To explore and analyze the effects of muscle strength training and endurance training in the treatment of patients with post-stroke spasticity.Method:102 patients with post-stroke spasticity who were admitted from February 2019 to August 2020 were selected and divided into experimental group and control group.The experimental group was given conventional rehabilitation training,and the experimental group was given muscle strength training and endurance training.The FMA,Berg,FIM and MAS scores of the two groups were compared before and after treatment.The shoulder and knee flexion and extension of the two groups were treated at an angular velocity of 60°/s.Results:Before treatment,there was no significant difference in FMA,Berg,FIM and MAS scores between the two groups of patients(P>0.05).After treatment,the FMA,Berg,and FIM scores of the two groups were significantly improved,and in the experimental group the FMA,Berg,and FIM scores of patients rose more significantly(P<0.05).After treatment,there was still no significant difference in MAS scores between the two groups(P>0.05).When the angular velocity was 60°/s,the shoulder and knee flexion and extension of the two groups before treatment.There was no significant difference in the level of biomechanical indexes(P>0.05).After treatment,the biomechanical indexes of shoulder and knee flexion and extension of the two groups were significantly improved,and the biomechanical indexes of shoulder and knee flexion and extension of the experimental group improved more significantly(P<0.05).The total effective rate of clinical curative effect in the experimental group was 96.08%compared with 82.35%in the control group(P<0.05).Conclusion:Muscle strength training combined with endurance training in patients with post-stroke spasticity can improve the motor function of the patients’limbs and maintain exercise.The balance function of this product can promote the recovery of daily life ability,which is worthy of promotion.
