MAIT细胞在慢性HBV感染患者中的表达及临床意义

Expression and clinical significance of MAIT cells in patients with chronic HBV infection

目的探讨黏膜相关恒定T(mucosal associated invariant T,MAIT)细胞在乙型肝炎病毒(hepatitis B virus,HBV)感染患者外周血中的表达及其临床意义。方法通过流式细胞仪对85例HBV感染患者和30名健康对照者进行外周血MAIT细胞表达水平进行检测。85例HBV感染患者包括慢性乙型肝炎(CHB)患者30例、乙型肝炎肝硬化(LC)患者25例(代偿期患者15例和失代偿期患者10例)、HBV相关慢加急性肝衰竭(HBV-ACLF)患者30例,比较HBV感染患者及健康对照者MAIT细胞的表达水平,同时比较LC患者中代偿期MAIT和失代偿期MAIT的表达情况;采用Pearson相关性检验分析MAIT细胞与HBV-ACLF患者终末期肝病模型(model for end-stage liver disease,MELD)评分关系,同时比较HBV-ACLF患者中好转组和死亡组MAIT细胞的表达情况。结果CHB患者外周血MAIT细胞水平为(3.68±0.91)%、LC患者外周血MAIT细胞水平为(2.18±0.12)%、HBV-ACLF患者外周血MAIT细胞水平为(1.42±0.80)%,均低于健康对照者(5.07±1.22)%(P均<0.05);在LC患者中,代偿期患者外周血MAIT细胞水平为(2.68±0.29)%,高于失代偿期患者(1.55±0.15)%(P<0.05);Pearson相关分析结果显示,HBV-ACLF患者外周血MAIT细胞水平与MELD评分呈负相关(r 2=0.6671,P<0.0001);在HBV-ACLF患者中好转组外周血MAIT细胞水平为(1.72±0.20)%,明显高于死亡组(0.89±0.08)%(P<0.05)。结论MAIT细胞水平的降低预示慢性HBV感染患者病情进展,提示MAIT细胞可能在HBV感染的发生发展过程中发挥重要作用。

Objective To investigate the expression and clinical significance of mucosal associated invariant T(MAIT)cells in peripheral blood of patients infected with hepatitis B virus(HBV).Methods The expression levels of MAIT cells in peripheral blood were detected by flow cytometry in 85 HBV infected patients and 30 healthy controls.85 patients with HBV infection included 30 patients with chronic hepatitis B(CHB),25 patients with HBV-related liver cirrhosis(LC)(15 patients with compensated and 10 patients with decompensated),and 30 patients with HBV-related acute-on-chronic liver failure(HBV-ACLF).The expression of MAIT cells in patients with HBV infection and healthy controls was compared.The expression of MAIT cells in compensated and decompensated was compared in patients with LC.Pearson correlation test was used to analyze the relationship between MAIT cells and model for end-stage liver disease(MELD)score,while comparing the expression of MAIT cells in the improvement group and the death group in patients with HBV-ACLF.Results The level of MAIT cells in peripheral blood of CHB patients was(3.68±0.91)%,LC patients was(2.18±0.12)%,HBV-ACLF patients was(1.42±0.80)%,which were lower than those of healthy controls(5.07±1.22)%(both P<0.05).In the LC patients,the level of MAIT cells in peripheral blood of patients with compensation[(2.68±0.29)%]was higher than that of the patients with decompensation(1.55±0.15)%(P<0.05).The results of Pearson correlation analysis showed that the levels of peripheral blood MAIT cells in patients with HBV-ACLF were negatively correlated with the MELD score(r 2=0.6671,P<0.0001),and the levels of peripheral blood MAIT cells in the improvement group[(1.72±0.20)%]was higher than that in the death group[(0.89±0.08)%](P<0.05).Conclusion Decreased levels of MAIT cells are predictive of disease progression in patients with chronic HBV infection,suggesting that MAIT cells may play an important role in the development of HBV infection.
