法医精神病伤残鉴定智能损害案例中血清微量元素的特点分析

摘要 目的探讨交通事故致颅脑损伤法医精神病伤残鉴定为智能损害的案例中血清微量元素的特点及其与伤残等级的关系。方法将交通事故致颅脑损伤经法医精神病伤残鉴定为智能损害的案例298例按韦氏成人智力测验(WAIS-RC)结果分为4组,边缘智力缺损组135例,轻度智力缺损组126例,中度智力缺损组26例,重度智力缺损组11例。对所有入组人员测定血清铜、铁、锌含量,并分析血清铜、铁、锌含量与伤残等级的关系。结果智力缺损边缘组与重度组血清铜比较,差异有统计学意义(P<0.05),血清铁各组间比较差异无统计学意义(P>0.05),血清锌比较边缘组和中度组、重度组及轻度组和重度组比较差异有统计学意义(P<0.05)。多元线性回归方程提示伤残等级与血清铜含量呈负相关,与血清锌含量呈正相关,而与血清铁含量无相关性。结论颅脑损伤法医精神病伤残鉴定案例者体内存在部分微量元素的失衡,伤残等级与血清铜、血清锌存在相关性。 Objective To explore the levels of serum microelements and their relations to disability grades in patients with intelligent damage in craniocerebral injury due to traffic accident according to forensic psychiatric disability identification.Methods A total of 298 patients with intelligent damage in craniocerebral injury due to traffic accident according to forensic psychiatric disability identification were divided into four groups by virtue of the scores of Wechsler adult intelligence test.There’re 135 cases in edge group,126 in mild group,26 in moderate group,and 11 in severe group.Blood samples were collected from all enrolled patients to determine the serum levels of copper,iron and zinc at fasting state.The correlations between serum levels of copper,iron,zinc and disability grades were analyzed.Results There is statistical significance on the differences between edge and severe groups at serum copper level(P<0.05);there in no statistical significance on the difference among all groups at serum iron level(P>0.05);there is statistical significance on the differences between edge and moderate groups,between edge and severe groups,and between mild and severe groups at serum zinc level(P<0.05).Multiple linear analysis suggested that there’s a negative correlation between disability grade and serum copper level;there’s no correlation between disability grade and serum iron level;there Vs a positive correlation between disability grade and serum zinc level.Conclusion Some microelements imbalanced in patients with craniocerebral injury due to traffic accident in forensic psychiatric disability identification,and there’re correlations between disability grades and serum copper,zinc levels.
