C-反应蛋白和脑脊液酸碱值在葡萄球菌感染肺炎患儿中的变化及意义 被引量:7

Expression of C-reactive protein and cerebrospinal fluid acid-base regulation in children with staphylococcal pneumonia

摘要 目的探究C-反应蛋白(CRP)和脑脊液酸碱等指标在葡萄球菌感染肺炎患儿中的表达情况。方法将2018年12月-2019年12月在浙江省立同德医院儿科接受治疗的48例葡萄球菌感染肺炎患儿及48例同期住院的非肺炎患儿作为研究对象,分别为研究组与对照组,对两组患儿C-反应蛋白(CRP)等炎性因子和脑脊液酸碱指标的表达差异进行对比。结果研究组48例患儿经痰液、粪便培养或呼吸道分泌物培养共分离出120株病原菌,其中19例患儿为混合感染,其中金黄色葡萄球菌72株,占比最多。研究组CRP、白介素-8(IL-8)以及降钙素原(PCT)、血氧分压(PO2)、血二氧化碳分压(PCO2)水平高于对照组(P<0.05);研究组的pH值低于对照组(P<0.05);多因素Logistic回归分析结果显示:CRP、PCT、PCO2与葡萄球菌感染肺炎存在正相性(P<0.05)。结论 C-反应蛋白和脑脊液酸碱指标在葡萄球菌感染中均存在明显表现,与患者患病与否存在明显的相关性,对患儿病情的诊断和治疗具有一定的借鉴作用。 OBJECTIVE To explore the expression of C-reactive protein and cerebrospinal fluid acid-base regulation in children with staphylococcal pneumonia. METHODS From Dec 2018 to Dec 2019, 48 children patients with staphylococcal pneumonia and 48 children patients without pneumonia who were hospitalized treated in the Department of Pediatrics of Tongde Hospital of Zhejiang Province were recruited as the study group and normal group, respectively. The differences of C-reactive protein(CRP) and other inflammatory factors and the PH value of cerebrospinal fluid acid-base expression between the two groups were compared. RESULTS A total of 120 strains of pathogenic bacteria were isolated from the sputum, fecal and respiratory secretions of 48 children in the study group. Among them, 19 cases were mixed infection, among of which 72 strains of were Staphylococcus aureus, accounteding for the largest proportion. The levels of inflammatory factors CRP, IL-8 and PCT in the study group were all significantly higher than those in the control group(P<0.05). The pH value of cerebrospinal fluid in the study group was significantly lower than that in the control group(P<0.05), but the levels of PO2 and PCO2 in the study group were both significantly higher than those in the control group(P<0.05). Multivariate ple factor Logistic regression analysis method showed that the factors CRP(OR=1.613), PCT(OR=1.383) and PCO2 were positively correlated with staphylococcal pneumonia. CONCLUSION C-reactive protein and cerebrospinal fluid acid-base play the important role in Staphylococcus infection have obvious performance, and there is obvious correlation between C-reactive protein and the incidence of the disease, which has a certain reference for the diagnosis and treatment of children’s disease.
