基于数据挖掘技术分析针灸治疗膝骨关节炎的选穴规律

Analysis of clinical acupoint selection rules in acupuncture treatment of knee osteoarthritis based on data mining technology

目的:分析总结针灸治疗膝骨关节炎的选穴规律,为临床治疗膝骨关节炎提供优化方案。方法:计算机检索中国知网(CNKI)、维普医药信息资源系统(VIP)、万方数据、中国生物医学文献服务系统(CBM)、PubMed、Embase、Springer、Cochrane Library数据库中的2014年11月1日至2019年11月1日符合纳入标准的针灸治疗膝骨关节炎的临床随机对照试验,以Microsoft Excel 2010软件建立数据库,采用描述性分析、关联规则分析及聚类分析方法,分析其选穴规律。结果:共纳入157篇文献的186条处方,使用频率最高的经脉、穴位分别为足阳明胃经和犊鼻穴,关联规则分析结果显示支持度最高的经脉及穴位组合分别为胃经-胆经、犊鼻-内膝眼。聚类分析结果显示,胃经、脾经、胆经聚为一类,犊鼻-内膝眼-梁丘-血海-足三里-阴陵泉-阳陵泉聚为一类。结论:临床针灸治疗膝骨关节炎遵循循经选穴与局部选穴原则,以胃经、经外奇穴、脾经、胆经穴位为主,犊鼻-内膝眼-梁丘-血海-阴陵泉-阳陵泉-足三里为核心穴位。

Objective:To analyze and summarize the rule of acupuncture and moxibustion in the treatment of knee osteoarthritis and to provide an optimal plan for the clinical treatment of knee osteoarthritis.Methods:The literatures in CNKI,VIP,Wanfang Data,CBM,PubMed,Embase,Springer and the Cochrane Library Data about acupuncture and moxibustion treatment of knee osteoarthritis from November 1,2014 until November 1,2019 were retrieved through computer,using Microsoft Excel 2010 software to establish a database,descriptive analysis,association rule analysis and cluster analysis were used to analyze the rule of hole selection.Results:A total of 186 prescriptions were included in 157 literatures.The most frequently used channel and acupoint were Yangming Stomach Channel of Foot and Dubi acupoint.The association rule analysis results showed that the most supported combination of channel and acupoint were Stomach with Gallbladder Channel,Dubi with Neixiyan acupoint.The results of cluster analysis showed that the Stomach,Spleen,Gallbladder Channel were grouped together,and Dubi,Neixiyan,Liangqiu,Xuehai,Zusanli,Yinlingquan,Yanglingquan acupoints were grouped together.Conclusions:In the treatment of knee osteoarthritis,acupuncture and moxibustion follows the principle of selecting points according to the channel and local acupoints,with the main acupoints from the Stomach,Spleen,Gallbladder Channel and non-meridian extra acupoint.The core acupoints can be Dubi,Neixiyan,Liangqiu,Xuehai,Yinlingquan,Yanglingquan and Zusanli.
