小针刀联合痹宁胶囊对湿热瘀阻、肝肾不足型痛风肾性关节炎临床疗效分析

Clinical Efficacy Analysis of Acupotomy Combined with Bining Capsule(痹宁胶囊)on Gouty Renal Arthritis with Dampness Heat and Blood Stasis,Deficiency of Liver and Kidney Type

目的观察痹宁胶囊联合小针刀对湿热瘀阻、肝肾不足型痛风肾性关节炎的临床疗效分析。方法选取2018年11月—2019年11月就诊于黑龙江中医药大学附属第一医院风湿科门诊的60例湿热瘀阻、肝肾不足型痛风肾性关节炎患者作为研究对象,采用随机数字表法将其分为针药组、中药组和针刀组,每组各20例。针刀组予以小针刀治疗,中药组口服痹宁胶囊,针药组予以痹宁胶囊联合小针刀治疗,2个月为1个疗程。观察3组患者治疗前后痛风肾性关节炎疼痛视觉模拟评分(VAS)、血尿酸(SUA)、24 h尿蛋白定量、C反应蛋白(CRP)和血沉(ESR)水平及中医疗效。结果治疗前3组各观察指标比较差异无统计学意义(P>0.05)。治疗后针药组VAS评分、SUA、24 h尿蛋白定量、CRP和ESR水平显著低于针刀组、中药组,中医疗效显著高于针刀组、中药组,差异有统计学意义(P<0.05)。结论小针刀联合痹宁胶囊治疗湿热瘀阻、肝肾不足型痛风肾性关节炎能够有效缓解患者关节疼痛,控制疾病发展,提高患者生活质量,值得临床推广。

Objective To observe the clinical effect of Bining Capsule(痹宁胶囊)combined with small needle scalpel on gouty nephritis of small needle knife.Methods From November 2018 to November 2019,60 patients with gouty renal arthritis of small needle knife were selected as the research objects.They were randomly divided into acupuncture group,traditional Chinese medicine group and acupotomy and medicine group,with 20 cases in each group.Acupotomy group was treated with small needle knife,traditional Chinese medicine group was treated with Bining Capsule,and acupuncture and medicine group was treated with Bining Capsule combined with small needle knife,with 2 months as a course of treatment.The pain viSUAl analogue score(VAS),serum uric acid(SUA),24-hour urine protein quantity,C-reactive protein(CRP)and ESR were observed before and after treatment.Results There was no significant difference among the three groups before treatment(P>0.05).After treatment,the VAS score,SUA,24-hour urine protein,CRP and ESR levels in the acupuncture and medicine group were significantly lower than those in the acupotomy group and the traditional Chinese medicine group,and the curative effect of traditional Chinese medicine was significantly higher than that of acupotomy group and traditional Chinese medicine group(P<0.05).Conclusion Small needle knife combined with Bining Capsule can effectively relieve the joint pain,control the development of disease and improve the life quality of patients with gouty nephritis of small needle knife.
